BEHAVIORAL CONTRACT

TO WHOM IT MAY CONCERN:
I, _____________________________________________, Grade ____________________________, hereby execute
a contract of good behavior with the administrator of Bulaoen East National High School, that I will not
commit any of the following offenses again:

MINOR OFFENSES
1. Absenteeism, tardiness, not attending classes nor coming to school late (4x) or more
2. Using cellphone or any gadget inside the classroom (RM No. 207 s 2018)
3. Loitering around the hallway and school vicinity/bleachers during class hour
4. Not wearing of proper uniform/tattered jeans/indecent attire/wearing of earrings(boys)/No
ID/hair highlights/longhair(boys)
5. Public Display of Affection (PDA)
6. Trespassing/jumping over perimeter fence
7. Not giving letter to parents
8. **Littering of garbage
9. **Vandalism writing on or destroying school property
MAJOR OFFENSES
10. Bullying (physical, verbal, cyber, emotional, mental) based on RA 10627 & DO 55 s. 2013
11. Carrying and concealing Deadly Weapon or Firearms
12. Cheating, dishonesty and stealing
13. Discourtesy/disrespectful towards fellow students, teachers and other authorities
14. Doing/Performing malicious acts/ Viewing Pornographic Videos or Photos
15. Drinking intoxicants or liquor/entering campus under the influence of liquor or drugs
16. Extortion: asking money from others
17. Forgery/imitation of signature or tapering with school records or transfer forms
18. Gambling of any sort of materials
19. Fighting: causing harm and injury to others
20. Gross misconduct/Unruly behavior inside the classroom or school premises
21. Immorality/Sexual Harassment or abuse, illicit relationships
22. Instigating, leading or participating in conceited activities resulting to stoppage of classes
23. Physical Assault: assaulting a teacher or any other school authority or fellow students
24. Smoking/Vaping inside the campus and use of illegal drugs (DO 40 s. 2012)
25. Preventing, threatening students or faculty members/school authorities from discharging their
duties or from attending classes or entering school premises
EXTREME OFFENSES
26. Using, possessing and selling prohibited drugs (RA 642)
27. Organizing or forming and joining fraternity or sorority and gangs
28. Hazing in any form or manner whether inside or outside the school premises

For Minor Offense/s committed, the following sanctions shall be imposed:


1st Offense: Verbal Warning and/ or on the spot school service**
2nd Offense: Verbal Warning with counseling
3rd Offense: Conference with parents/guardians and advisers; signing of Behavior Contract

For Major Offense/s committed, the following sanctions shall be imposed:


1st Offense: Verbal Warning with counseling
2nd Offense: Conference with parents/guardians and advisers; signing of Behavior Contract
3rd Offense: Suspension which will not exceed three (3) days
4th Offense: Refer to Modular/Advise to leave or to transfer

For Extreme Offense/s committed, the following sanctions shall be imposed:


1st Offense: Conference with parents/guardians and advisers; signing of Behavior Contract, Suspension
for seven (7) days
2nd Offense: Suspension for more than seven (7) days but not more than one (1) year- refer it to SDS for
approval
3rd Offense: Suspension for more than one (1) year or more/ Expulsion- refer it to the Secretary for
approval
I further promise that if commit an such offense, I could be subjected or bounded with measures
indicated in the School Rules and Regulations.
