AFFIDAVIT OF ONE AND THE SAME PERSON

I, ANNA MAE R. RAMORES, of legal age, Filipino, single and presently


residing at Nangka St., Sampaloc, Tanay, Rizal, on oath hereby depose and state
that:

1. I am the mother of FREYA HOPE RAMORES ALARAS who was born on


28 December 2010 in Imus City, Cavite.

2. As per my Certificate of Live Birth and in all my public records, my last


name is “RAMORES”.

3. However, in my minor daughter’s Certificate of Live Birth issued by the


Philippine Statistics Authority and the City Civil Registrar’s Office, the following
entries were erroneously entered and should be corrected as follows: child’s
Middle Name and mother’s Last Name from “RAMOREZ” to “RAMORES”.

3. I hereby certify that FREYA HOPE RAMOREZ ALARAS and FREYA


HOPE RAMORES ALARAS refer to one and the same person.

4. The above discrepancies in my daughter’s Certificate of Live Birth were


brought upon by clerical errors and not by any intention to deceive or mislead the
public.

5. I am executing this Affidavit to attest to the truth of the foregoing and for
whatever legal purposes this may serve.

IN WITNESS WHEREOF, I have hereunto signed this Affidavit this August
