Task-1

Possible uninsured cost for the Northtown Health Club accident

Fines (cost) resulting from prosecution by enforcement authority – As the Fitness


instructor broken his leg in an accident so there may be a chance of prosecution action by
enforcement authority resulting in warning notice, prohibition notice, punitive action, fines,
etc. ✔

Worker sick pay (cost) – Until fitness instructor not recovered and return to duty his sick
pay cost has to be bear by NHC. ✔

Staff recruitment cost – Until fitness instructor not available in duty his post to be fulfilled
by new recruitment this NHB need to bear the cost for it. ✔

Increases in insurance premiums cost resulting from the accident – As there occurs an
accident which will increase insurance premium also. ✔

Compensation cost not covered by the insurance policy – If fitness instructor demand for
extra compensation cost except insurance policy covered has to be bear by NHC. ✔

Legal representation cost following any compensation claim – Company Legal


representative needs to represent company’s side in court cost for it will be bear by NHC. ✔

During audit results will lead to loss of goodwill and poor corporate image - which leads to
less helpful in tender bidding Recruitment cost of replacement staff. ✔

Training cost of replacement staff – As fitness trainer not available new recruitment needs
to provide training as per norms cost shall be bear by NHC. ✔

Extra overtime payments – As we observed workers were engaged for overtime due to lack
of workers and work hard also cost has to be bear by NHC. ✔

Lost time for other employees, such as first-aid staff, to help the injured person – As
fitness instructor needs to provide first aid until ambulance arrived trained first aider need
to provide their services, first aider staff cost has to be bear by NHC. ✔

Additional administration time incurred Due to accident- there may be investigation and
other action may be there so there may be a need of additional administrative time required
for it. ✔

Lower employee morale possibly leading to reduced productivity – Worker after an


accident may feel demoralised and demotivated due to unsafe work practices this will affect
moral of the worker which can’t be easily calculated. ✔
Loss of goodwill and a poor corporate image – As accident happened the Northwest Health
club reputation loss and good will loss will lead to affect tender bidding also.

Task -02
Management failure contributed to accident-

Not appointing maintenance engineers – As we observed that HCM asked fitness instructor
for maintenance to repair the machine there we found that there is no provisions of
maintenance engineer to perform the task of maintenance. ✔

Faulty equipment need continuous repaired – As there four machine already in use which
were need repaired continuously at Northwest Health Club with do not use sign placed
there shows management failure to provide safe machines. ✔

Increase in near misses – During this year increase in health & safety near misses incidents
here has been reported across most of sites in ELC which leads to accident. ✔

Not having up to date health & safety policy – IN Elite Health Club health & safety policy
were not reviewed from last 10 years when they made it this show lack of management
poor commitment towards H&S policy. ✔

Poor communication of H&S policy – a) As while interviewing with the workers they replied
management policy is not available to when they unsure about a potential safety concern
they search in intranet to get solution. So management H&S policy not available there. ✔

b) While interviewing with workers the H&S officer noticed that workers replied that they
had read it but not understand. ✔

Poor response of management towards reported accident & incident and near misses – As
HCM told to Health and safety officer about they tried to thoroughly report about accident
and incident and near misses to management. They rarely take any action on it this shows
poor approach of management.

HCM unawareness of inspection process – When H&S officer asked about inspection
records the HCM given maintenance records these shows HCM poor approach an
unawareness about poor Inspection record keeping procedure. ✔

Not updating training records – As H&S officer during observe that there is no maintained
training records for three years these shows seriousness of management towards training
process. ✔

Not having proper training procedure – As new workers were trained by long serving
workers and they occasionally maintain their records of the training shows that company
failed to maintain the appropriate procedure. ✔
Not having sufficient workers – As Workers explaining about overtime carried out by the
workers we observed that there is a lack of sufficient manpower. ✔

No trained Inspection team members – A maintenance workers raise his issue about
inspection that they are unable to ask for help as no one have been trained to inspect them.

Not updating fire drill records – While H&S officer during auditing found that HCM
surprised when see the fire drill records and shows his concern saying that they perform the
last drill but not confirm when they do it.

Task – 03
Delayed action affected the gathering information stage of the investigation.
Secure the scene – After the accident it was noticed that they removed exercise machine
from site this could have affected investigation process to secure the scene. ✔

Identify witnesses – As audit was made after a long time of accident there may be a chance
of missing of witness who might be left the Northen Health Cub. ✔

Collect factual information – If any enforcement action taken against the Northwest Health
Club there is chance of legal action because of an accident, they will have to face punitive
action as they have disturbed the accident scene and collect the factual information.

Interview witnesses – As we are not sure as the time passes witness may not remember the
accident and forgot the accident sequence. ✔

Examine documents – As NHC not perform inspection due to not having competent person
for it and procedure for it✔

Factual information – As machine was overloaded with the weight of plates before carried
out checking of machine by fitness instructor information were missed due to delay in
gathering the information. ✔

b) The fitness instructor got injured due to faulty locking mechanism of exercise machine
and not put it in their idle position these information could be missed after delaying in
gathering the information. ✔
Task – 04
What is positive approach taken to the audit
New CEO, of Elite Health group employed a health and safety officer as audit for conducting
and reviewing annual health and safety for every health club. ✔
New appointed H&S officer is having high competency level as having 8 years of
experience in auditing and recent charter ship as H&S professional. ✔
After arrival at NHC auditor taken a tour around the site of NHC accompanied by HCM who
describe about accident happened one month ago. ✔
Auditor while visit of the gym also looked at existing four machines with do not sign and
noticed NHC purchased 07 years ago, now become out dated.
During auditing in first day HCM was inquired for H&S documentation observed
organizational failure as they not responded reports of accident investigation and near
misses done by NHC personnel. ✔
Auditor during a tour of the gym asked HCM for increasing number of near misses involving
exercise machine’s investigation records which they failed to provide. ✔
Auditor asked HCM to show accident investigation record but instead of providing it HCM
shown him exercise machine maintenance record.
During auditing on day 02 auditor discovered that there is not having training records and
proper provisions for providing training. ✔
During auditing on day 02 auditor discovered that there is no specific procedure for risk
assessments, accident, near-misses record keeping. ✔
During auditing on day 02 auditor discovered that fire drills were performed but failed for
proper documentation of it and also shows unawareness of frequency of fire drill. ✔
Interviewing the workers – Auditor during audit talked each section of workforce separately
for identifying the working conditions. ✔
