Dear Respondent:

The researchers invite you to participate as one of the respondents in their proposed
research study titled "PERCEPTION OF SENIOR HIGH SCHOOL STUDENTS ON HANDS-ON
LABORATORY ACTIVITY." Its main objective is to understand the perception of senior high
school grade 11 STEM students of Sangali National High School on hands-on laboratory activity.

During the initial phase of the study, you will be asked to provide essential information as
part of the profiling process. In the second phase, the researchers will be conducting a face-to-face
semi-structured interview that includes a range of topics regarding perspectives and hands-on
laboratory activities to collect the necessary data for the study. This interview session includes (12)
interview questions to be answered by the respondent.

Your participation in this research study is completely voluntary. You have the right to
decline participation or withdraw from the study at any time, and you may choose not to answer
any interview questions that you are not comfortable with. All information provided by the
respondent will be treated with utmost confidentiality to safeguard your identity and rights.

The data collected from participants will be aggregated in such a way that individual
identities and responses remain anonymous. The published results will not disclose your personal
information, ensuring your privacy and compliance with Republic Act No. 10173, also known as
the Data Privacy Act of 2012. Only the researchers will have access to your individual responses in
this interview questionnaire, and no names or other personally identifiable information will be
disclosed or published. Your data will be handled with the highest level of privacy and data
protection.

If you agree to take part in this study, it will be greatly appreciated. It is estimated that
completing this Interview Session will require approximately 10 to 15 minutes of your time.
