CRIME SCENE INVESTIGATION REPORT

MEMORANDUM

FOR : Chief of Police

FROM : Investigator-on-Case

SUBJECT : Crime Scene Investigation Report re Rape with Homicide

DATE :

1. Reference: Philippine National Police (PNP)

2. ICOW the above reference, respectfully submitted is the Report on Crime


Scene Investigation conducted on October 14, 2024 at 3:30 pm at the Saint
Theresa College of Tandag City Inc. for alleged Rape with Homicide incident.

3. The following are the significant activities and information:

a. That the First Responders led by Vergara, Civran R. arrived at the


crime scene o/a 3:30 pm on October 14, 2024, and was able to secure and
establish the security cordon o/a 3:40 pm on October 14, 2024; (Please see
attached FR Request of Demontar, Reiner R. CLO).

b. That the Investigator-on-Case arrived at the crime scene on/about 3:50


pm on October 14, 2024 and thereafter assumed responsibility over the crime
scene and conducted inquiry on the incident; (see attached Investigation CSI Form)

c. That members of the SOCO TEAM (if applicable) led by Briones, Izaak
Steffene T. arrived at the crime scene o/a 4:20 pm on October 14, 2024 and
conducted forensic investigation and collected all potential pieces of evidence; and

d. That the crime scene was formally released to PCpl Tamaca, Rom
Adrian of Tandag City Police Station and security cordon was lifted on/a October
14, 2024 at 5:00 pm.

4. Attached are the FR’s Report, Investigator’s CSI Form and SOCO reports
for your reference which will form as an integral part of the Case Folder of Rape
with Homicide incident.
