Eligibility
Full-time staff members with a regular schedule of 30+ hours per
week are eligible for the benefits described in this guide, unless
otherwise stated.

When Benefits Become Effective


Coverage for most benefit plans are effective on your date of hire. Part-
time staff members are only eligible for vision, voluntary insurance, and
retirement benefits.

Eligible Dependents
Your dependents are eligible to participate in Apple's benefit plans. Your
eligible dependents include*:
· A spouse to whom you are legally married.
· A
 dependent child under age 26. Coverage for all benefits will
terminate at the end of the month of the dependent’s 26th
birthday. Coverage may be extended past the age of 26 for disabled
dependents. Dependent children can include natural, adopted
children, and stepchildren.
Coverage for eligible dependents generally begins on the same day
your coverage is effective. Completed enrollment serves as a request for
coverage and authorizes any payroll deductions necessary to pay for that
coverage.

Spousal Surcharge
If an employee enrolls their spouse in our medical insurance and that
spouse has access to enroll in insurance through their employer, a $35
per pay period surcharge is added to the premium.

Newly Hired/Eligible Staff Members

New hires and newly eligible staff members MUST complete enrollment
even if choosing to waive coverage in order to provide beneficiary
information for your company-paid life insurance.

Pre-Tax Benefits: Section 125


Apple's benefit plans utilize Section 125. This enables you to elect to pay premiums for health, dental, vision and flexible
spending account coverage on a pre-tax basis. When you use pretax dollars, you will reduce your taxable income and have
fewer taxes taken out of your paycheck. Under Section 125, you can actually have more spendable income than if the same
deductions were taken on an after tax basis.
Pre-tax Note: When you pay for your dependent’s benefits on a pre-tax basis you are certifying that the dependent meets
the IRS’ definition of a dependent. [IRC §§ 152, 21 (b)(1) and 105(b)]. Children/spouses that do not satisfy the IRS’ definition
will result in a tax liability to you, such as changing that dependent’s election to a post-tax election, or receiving imputed
income on your W-2 for the dependent’s coverage that should not have been taken on a pre-tax basis.

Benefit Changes
The benefit elections you make during open enrollment or as a new
hire will remain in effect for the entire plan year. You will not be able to
change or revoke your elections once they have been made unless a life
event status change occurs.
For purposes of health, dental, vision and flexible spending accounts,
you will be deemed to have a life event status change if:
· your marital status changes through marriage, the death of your
spouse, divorce, legal separation, or annulment;
· your number of dependents changes through birth, adoption,
placement for adoption, or death of a dependent;
· you, your spouse or dependents terminate or begin employment;
· your dependent is no longer eligible due to attainment of age;
· you, your spouse or dependents experience an increase or reduction
in hours of employment (including a switch between part-time and
full-time employment; strike or lock-out; commencement of or return
from an unpaid leave of absence);

Medical Coverage
Provider: Anthem Group: 0B3825
Apple is proud to offer you a choice between four different medical
plans, all of which feature access to Anthem providers including
specialists, without requiring referrals. The first plan utilizes the
Anthem HMO Network while the other three utilize the
Anthem KeyCare Network. You are not required to choose a
primary care physician. The plans include comprehensive medical care
and prescription drug coverage and offer many resources and tools
to help you maintain a healthy lifestyle.

Anthem HMO

The Anthem HMO plan is a Health Maintenance Organization plan, or


HMO for short. This plan features a $0 deductible for in-network
providers which means you are only responsible for copays and

Generic drugs are made with the same active ingredients and produce the same effects in the body as their brand-name
equivalents. That’s because they’re held to the same federal standards for safety and performance as the brand names. Because
they’re not branded, generics can sell for 30 percent to 80 percent less than their brand-name equivalents.

the cost is your responsibility when in the deductible phase. This could potentially
save you a lot money.
Regardless of which plan you decide to enroll in, we encourage you to download
and use the GoodRx Mobile App to help you save on your prescription drug
costs. Prices for prescription drugs vary widely between pharmacies. The cost of
a prescription may differ by more than $100 between two pharmacies across the
street from each other.
GoodRx doesn’t sell the medications, they will tell you where you can get the best
deal on them. GoodRx will show you prices, coupons, discounts and savings tips
for your prescription at pharmacies near you. Out of pocket dollars do not go

Health Reimbursement Arrangements


Eligible if enrolled in: Anthem HRA $1500 or Anthem HRA $3000

If you enroll in the Anthem HRA $1500 or Anthem HRA $3000, a Health Reimbursement Arrangement (HRA) will be created
for you through Anthem Act Wise. An HRA is a financial account that you can use to pay for qualified health care expenses,
as defined by the Internal Revenue Service. The account is funded and owned by Apple Federal Credit Union. Unlike an HSA or
FSA, you cannot contribute funds to your Health Reimbursement Arrangement.
The employer contribution is 50% but after Jan. 1, it’s prorated and can be spent by you on medical expenses, and
even though Apple owns and funds the HRA, think of it as your money. By doing so, you’ll realize that spending your HRA funds
wisely can help you save money.

How the claim and billing process works


• You will use your HRA debit card to pay
If the service is covered and you used a network doctor or facility, Anthem applies the network discount. Preventive
care is covered at 100% and does not use HRA funds. If the claim is not for preventive care you will use your HRA debit
card to pay your bill until all of your HRA has been spent.

• Save your proof to show that you incurred an HRA-eligible expense


IRS rules requires that you used your debit card for an HRA-eligible expense and occasionally, Anthem will request that
you submit proof to substantiate your expense. Proof of purchase could include the following:
- An explanation of benefits (EOB) statement (available from Sydney app or Anthem.com)
- A receipt

Health Savings Accounts

If you enroll in the Anthem HSA $3000 a Health Savings Account (HSA) will be created for you through Anthem Act Wise.
An HSA is a financial account that you can use to accumulate tax-free funds to pay for qualified health care expenses, as defined by
the IRS. The account is similar to a traditional savings account with a debit card. The money in the account is owned by you and is
fully portable. Funds can accumulate over time and roll over each year. If you use the funds for qualified health care expenses, you will
pay no taxes. If you use the money for other expenses, you will pay a tax and a penalty fee.

The two plans are a Premium option and a Basic option. Both cover preventive
and diagnostic services at 100% up to R&C. Once you meet your deductible, the

Flexible Spending Accounts (FSA’s) offer another way to save money on health
care and dependent care expenses. You may submit expenses incurred by any
of your dependents, whether or not they are covered by the insurance plans
you have through your employer. Staff members need not be enrolled in either
medical plan to participate in FSAs.
If you enroll, you fund the accounts via a payroll deduction each pay period.
Money that you contribute to your FSAs is not subject to social security taxes,

Short-Term Disability
Provider: Guardian
To ensure your income will continue if you are unable to work due
to a disability that extends for more than 14 consecutive days, Apple
provides short-term disability (STD) through Guardian. Benefits are
payable for a non-occupational injury or illness that keep you from
performing the normal duties of your job. If a medical condition is job-
related, it is considered Workers’ Compensation rather than STD.
Eligibility Waiting Period: 120 days of service
Benefits Start After: 14 days
Benefit Amount: 66.67% of pay up to $2,000 per week
Benefit Duration: Up to 11 weeks

Long-Term Disability
Provider: Guardian
Long-Term Disability (LTD) insurance helps replace a portion of your income if you are disabled for an extended period of
time. Eligibility for long-term benefits are generally defined as, due to sickness or accidental injury which you are receiving
appropriate care and treatment; are complying with your treatment requirements and unable to earn more than 80% of
your predisability earnings.
Eligibility Waiting Period: 120 days of service
Benefits Start After: 90 days
Benefit Amount: 66.67% of pay up to $10,000 per month
Benefit Duration: The later of your SSNRA* or the Maximum Benefit Period.
*SSNRA means the Social Security Normal Retirement Age in effect under the Social Security Act on the Policy Effective Date.

Pre-Existing Condition Limitations


The carrier will not pay benefits for any period of Disability caused or contributed to by, or resulting from, a Pre-existing
Condition. A “Pre-existing Condition” means any Injury or Sickness for which you incurred expenses, received medical
treatment, care or services including diagnostic measures, took prescribed drugs or medicines, or for which a reasonable
person would have consulted a Physician within 3 months before your most recent effective date of insurance.
The Pre-existing Condition Limitation will apply to any added benefits or increases in benefits. This limitation will not apply
to a period of Disability that begins after you are covered for at least 12 months after your most recent effective date of
insurance, or the effective date of any added or increased benefits.

Pension Retirement Plan


Pension Plan Provider: Pentegra

Apple contributes generously to a defined benefit pension plan for


all qualified employees. A pension is a career reward plan because
pension benefits are based on salary and years of service with your
employer, generally, the longer you work for your employer the greater
your benefit will be. And while the value of this benefit may not be
fully visible in the early years of your career, the longer you work for
your employer, the more significant it will become. It is Apple’s way of
saying “thanks” for your important contributions during your career.

· Eligible after 1 year of service and 20 ½ years old


· No cost to staff members. This benefit is 100% Employer Paid
· Paid to you in retirement; amount is based on service and salary

 -year cliff vesting schedule. According to this schedule, you will
become 100% vested after 5 years of service, and 0% vested before

Paid Time Off


Apple believes that staff members should have opportunities to enjoy time away from work to help relax and balance their lives.
For this reason, all staff members are encouraged to take time off from work.

Full-Time Staff Members

Eligible staff members will accrue 3.69 hours per paycheck of vacation leave. Accrual rate increases with years of employment on
the staff member’s work anniversary date. A maximum of 96 bonus vacation hours may be accrued.

Sick
The availability and use of sick leave is an extremely valuable staff privilege which is available to full-time staff members. Sick leave
is designed to provide income for hours missed from work due to personal illness; it may not be used in place of other types of
paid time off such as Vacation Leave or Personal Leave.
Eligible staff members will accrue 3.69 hours per paycheck of sick leave. There is no maximum amount of sick leave accrual.
Sick leave may be used only for an absence due to a staff member’s illness or injury or that of a family member who resides in the
staff member’s household. It may also be used for a staff member’s medical or dental appointments for a family member who
resides in the staff member’s household.

Personal Leave
In addition to vacation and sick leave, full-time staff members will also receive 16 hours of personal leave per calendar year, which
may be used at their discretion, pending supervisory approval. Personal leave hours are earned on January 1 of each year and
reported in the first pay period of the year which includes January 1. Personal leave hours must be used in the year in which they
are earned, and are not carried forward into the next year, meaning this is a use it or lose it leave. Staff members hired in the first
quarter of the calendar year will receive 16 personal leave hours for the year. Staff members hired in the second quarter of the year
will receive 8 personal leave hours for the year. Staff members hired in the third and fourth quarter of the year receive no personal
leave hours for that year.

Rollover Limit – At the end of the calendar year, accumulated vacation leave in excess of the maximum of 240 hours will be
paid out at a staff member’s current rate of pay in his or her final paycheck of the year as it will not be carried forward into the
next calendar year.

Sellback Option – Full-time staff members may sell back accrued vacation leave in excess of 80 hours at 100 percent of their
regular hourly rate at the time of the request. Leave may be sold back in any biweekly pay period. Staff members who wish to sell
back leave must complete and submit to People and Culture a Leave Sell Back Request Form, by the last Friday of a biweekly pay
period to receive payment for the upcoming pay date. Sold-back leave is subject to income taxes and withholding as with any income.

Part-Time Staff Members

Part-time Staff Members’ vacation accrual is calculated as the percentage of time worked multiplied by the regular benefit. The
benefit increases with each year of employment. For example:
· A staff member working 25 hours per week with 1 year of employment would be eligible for 7.5 vacation days
(25⁄40 x 12 days)
· A staff member working 25 hours per week with 5 years of employment would be eligible for 10 vacation days
(25/40 x 16 days)

Holidays
The credit union generally observes the same paid holidays as the Federal Reserve. In January of each year, P&C will publish a
schedule of the holidays, including the dates on which they will be observed. Usually, Apple FCU observes the following eleven
(11) paid holidays:

• Martin Luther King Day • Columbus Day

• President’s Day • Veterans Day

• Memorial Day • Thanksgiving Day

• Juneteenth Day • Christmas Day

• Independence Day
