Eligibility
Full-time staff members with a regular schedule of 30+ hours per
week are eligible for the benefits described in this guide, unless
otherwise stated.
Eligible Dependents
Your dependents are eligible to participate in Apple's benefit plans. Your
eligible dependents include*:
· A spouse to whom you are legally married.
· A
dependent child under age 26. Coverage for all benefits will
terminate at the end of the month of the dependent’s 26th
birthday. Coverage may be extended past the age of 26 for disabled
dependents. Dependent children can include natural, adopted
children, and stepchildren.
Coverage for eligible dependents generally begins on the same day
your coverage is effective. Completed enrollment serves as a request for
coverage and authorizes any payroll deductions necessary to pay for that
coverage.

Spousal Surcharge
If an employee enrolls their spouse in our medical insurance and that
spouse has access to enroll in insurance through their employer, a $35
per pay period surcharge is added to the premium.
New hires and newly eligible staff members MUST complete enrollment
even if choosing to waive coverage in order to provide beneficiary
information for your company-paid life insurance.
Benefit Changes
The benefit elections you make during open enrollment or as a new
hire will remain in effect for the entire plan year. You will not be able to
change or revoke your elections once they have been made unless a life
event status change occurs.
For purposes of health, dental, vision and flexible spending accounts,
you will be deemed to have a life event status change if:
· your marital status changes through marriage, the death of your
spouse, divorce, legal separation, or annulment;
· your number of dependents changes through birth, adoption,
placement for adoption, or death of a dependent;
· you, your spouse or dependents terminate or begin employment;
· your dependent is no longer eligible due to attainment of age;
· you, your spouse or dependents experience an increase or reduction
in hours of employment (including a switch between part-time and
full-time employment; strike or lock-out; commencement of or return
from an unpaid leave of absence);

You may also be permitted to change your elections for health coverage
under the following circumstances:
· a court order requires that your child receive accident or health
coverage under this plan or a former spouse’s plan;
· you, your spouse or dependent become entitled to Medicare or
Medicaid;
· you have a Special Enrollment Right;
· there is a significant change in the cost or coverage for you or your
spouse attributable to your spouse’s employment.
For purposes of all other benefits under the plan, you will be deemed
to have a life event status change if the change is on account of and
consistent with a change in status, as determined by the plan administrator,
in its discretion, under applicable law and the plan provisions.

You must update your elections within 30 days of your life event status change or you will not be able to make changes
until the next annual open enrollment. If adding or removing dependents, you are required to submit specific documents to Apple
FCU. The change will be inactive until proper documentation is received and approved. For assistance processing life event status

Medical Coverage

Apple is proud to offer you a choice between four different medical
plans, all of which feature access to Anthem providers including
specialists, without requiring referrals. The first plan utilizes the
Anthem HMO Network while the other three utilize the
Anthem KeyCare Network. You are not required to choose a
primary care physician. The plans include comprehensive medical care
and prescription drug coverage and offer many resources and tools
to help you maintain a healthy lifestyle.
Anthem HMO
Prescription Coverage

Your prescription drug benefit is part of your medical plan. The Rx Mail Order Program
prescription drug formulary generally lists many drugs and ranks them in

Generic drugs are made with the same active ingredients and produce the same effects in the body as their brand-name
equivalents. That’s because they’re held to the same federal standards for safety and performance as the brand names. Because
they’re not branded, generics can sell for 30 percent to 80 percent less than their brand-name equivalents.
If you enroll in the Anthem HRA $1500 or Anthem HRA $3000, a Health Reimbursement Arrangement (HRA) will be created
for you through Anthem Act Wise. An HRA is a financial account that you can use to pay for qualified health care expenses,
as defined by the Internal Revenue Service. The account is funded and owned by Apple Federal Credit Union. Unlike an HSA or
FSA, you cannot contribute funds to your Health Reimbursement Arrangement.
The employer contribution is 50% but after Jan. 1, it’s prorated and can be spent by you on medical expenses, and
even though Apple owns and funds the HRA, think of it as your money. By doing so, you’ll realize that spending your HRA funds
wisely can help you save money.

If you enroll in the Anthem HSA $3000 a Health Savings Account (HSA) will be created for you through Anthem Act Wise.
An HSA is a financial account that you can use to accumulate tax-free funds to pay for qualified health care expenses, as defined by
the IRS. The account is similar to a traditional savings account with a debit card. The money in the account is owned by you and is
fully portable. Funds can accumulate over time and roll over each year. If you use the funds for qualified health care expenses, you will
pay no taxes. If you use the money for other expenses, you will pay a tax and a penalty fee.
* Apple's contribution to your HRA/HSA is prorated based on the date of entry into the plan.
Note: As a taxpayer, it is your responsibility to ensure that your HSA contributions do not exceed the maximum possible for your specific tax situation.

Dental Coverage

You have a choice between two dental plans, both administered by Guardian.
The two plans are a Premium option and a Basic option. Both cover preventive
and diagnostic services at 100% up to R&C. Once you meet your deductible, the
plans provide coverage for a range of other services, including minor and major
restorative.

Short-Term Disability
Provider: Guardian
To ensure your income will continue if you are unable to work due
to a disability that extends for more than 14 consecutive days, Apple
provides short-term disability (STD) through Guardian. Benefits are
payable for a non-occupational injury or illness that keep you from
performing the normal duties of your job. If a medical condition is job-
related, it is considered Workers’ Compensation rather than STD.
Eligibility Waiting Period: 120 days of service
Benefits Start After: 14 days
Benefit Amount: 66.67% of pay up to $2,000 per week
Benefit Duration: Up to 11 weeks
Long-Term Disability
Provider: Guardian
Long-Term Disability (LTD) insurance helps replace a portion of your income if you are disabled for an extended period of
time. Eligibility for long-term benefits are generally defined as, due to sickness or accidental injury which you are receiving
appropriate care and treatment; are complying with your treatment requirements and unable to earn more than 80% of
your predisability earnings.
Eligibility Waiting Period: 120 days of service
Benefits Start After: 90 days
Benefit Amount: 66.67% of pay up to $10,000 per month
Benefit Duration: The later of your SSNRA* or the Maximum Benefit Period.
*SSNRA means the Social Security Normal Retirement Age in effect under the Social Security Act on the Policy Effective Date.
Voluntary Benefits
Provider: Guardian
The following Voluntary Benefits can complement existing medical coverage and help fill financial gaps caused by out-of-pocket
expenses such as deductibles, co-payments, and non-covered medical services. Benefits are paid regardless of what is covered by
medical insurance. Payments are made directly to you, to spend as you choose.

Eligible staff members will accrue 3.69 hours per paycheck of vacation leave. Accrual rate increases with years of employment on
the staff member’s work anniversary date. A maximum of 96 bonus vacation hours may be accrued.
Sick
The availability and use of sick leave is an extremely valuable staff privilege which is available to full-time staff members. Sick leave
is designed to provide income for hours missed from work due to personal illness; it may not be used in place of other types of
paid time off such as Vacation Leave or Personal Leave.
Eligible staff members will accrue 3.69 hours per paycheck of sick leave. There is no maximum amount of sick leave accrual.
Sick leave may be used only for an absence due to a staff member’s illness or injury or that of a family member who resides in the
staff member’s household. It may also be used for a staff member’s medical or dental appointments for a family member who
resides in the staff member’s household.
Personal Leave
In addition to vacation and sick leave, full-time staff members will also receive 16 hours of personal leave per calendar year, which
may be used at their discretion, pending supervisory approval. Personal leave hours are earned on January 1 of each year and
reported in the first pay period of the year which includes January 1. Personal leave hours must be used in the year in which they
are earned, and are not carried forward into the next year, meaning this is a use it or lose it leave. Staff members hired in the first
quarter of the calendar year will receive 16 personal leave hours for the year. Staff members hired in the second quarter of the year
will receive 8 personal leave hours for the year. Staff members hired in the third and fourth quarter of the year receive no personal
leave hours for that year.
Rollover Limit – At the end of the calendar year, accumulated vacation leave in excess of the maximum of 240 hours will be
paid out at a staff member’s current rate of pay in his or her final paycheck of the year as it will not be carried forward into the
next calendar year.
Sellback Option – Full-time staff members may sell back accrued vacation leave in excess of 80 hours at 100 percent of their
regular hourly rate at the time of the request. Leave may be sold back in any biweekly pay period. Staff members who wish to sell
back leave must complete and submit to People and Culture a Leave Sell Back Request Form, by the last Friday of a biweekly pay
period to receive payment for the upcoming pay date. Sold-back leave is subject to income taxes and withholding as with any income.

Part-time Staff Members’ vacation accrual is calculated as the percentage of time worked multiplied by the regular benefit. The
benefit increases with each year of employment. For example:
· A staff member working 25 hours per week with 1 year of employment would be eligible for 7.5 vacation days
(25⁄40 x 12 days)
· A staff member working 25 hours per week with 5 years of employment would be eligible for 10 vacation days
(25/40 x 16 days)
Holidays
The credit union generally observes the same paid holidays as the Federal Reserve. In January of each year, P&C will publish a
schedule of the holidays, including the dates on which they will be observed. Usually, Apple FCU observes the following eleven
(11) paid holidays:
