Unit 3 requires candidates to teach a lesson which is to be observed by the mentor. This form is
designed to support the observation process.

Mentors should:
 Read pages 17 – 20 of “A guide for Mentors” available on PDQ Connect
 Review the Exemplar portfolio/s on PDQ Connect to help them understand the requirements of
this form
