ULO 4: Work individually and in a team to analyze, interpret, and address
problems in safety and sustainability assessment.

Complete the below team work evaluation forms. This evaluation is confidential and should
not be discuss with other team members. An example on how to fill the form is given in
page 6 and 7.

Student will get 0 mark if this assessment is not submitted. Student marks will be deducted
accordingly if the form is fill incorrectly.

The convener reserves the right to change the rating/vary the marks if there are strong
evidence confirming the statement.
