CULT/OCCULTISM AND TO BE OF GOOD CONDUCT

Matriculation Number ……………………………………………… do hereby make this oath and declare as follows:

1. THAT I am a student of Mountain Top University, Prayer City; Ibafo in Ogun State of Nigeria.

2. THAT prior to my admission: -


(a) I was not a member of any secret cult or society nor was I involved in any secret cult
practices and never involved in the consumption of alcohol was, hard drugs, cannabis,
cigarettes, tobacco and substance abuse

OR
(b) I was a member of …………………………….……………….………………………………. secret cult in
…………………………………………….………………………………………….…….………. (Name of
University/Polytechnic/College of) AND I was involved in the taking of alcohol, hard drugs,
cannabis, cigarettes, tobacco and substance abuse but have been completely rehabilitated
and weaned from it (TICK & UNDERLINE AS APPLICABLE)

3. THAT I have not been influenced into attending, watching or participating in any Secret
Society/Cult Meetings organized under any umbrella whatsoever or consuming alcohol, hard
drugs, cigarettes, tobacco, cannabis and substance abuse;

4. THAT I will not belong or get enticed into joining, organizing, participating or attending meeting
of any secret society or secret cult activities and will not consume or participate in the
consumption of; or lure and encourage any person to consume alcohol, hard drugs, cannabis,
cigarettes, tobacco and substance abuse within and outside of Mountain Top University, Prayer
City;

5. THAT I undertake to expose and report any move by any individual or group(s) that make
attempt to lure me to join or to participate or to facilitate/encourage any person to join Secret
Cult or Secret Society OR to purchase, possess, consume or join in the consumption of alcohol,
hard drugs, cannabis, cigarettes, tobacco, or engage in substance abuse; within and outside of
Mountain Top University and the Prayer City;
6. THAT such moves if any exist shall be reported by me to either the Registrar, Dean of Students,
Chief Security Officer or the Management within a reasonable time of its occurrence or as most
practicable as facts and circumstance permit;

7. THAT as an individual if I had in the past belonged to or had been involved at any stage of my
educational career in Secret Society/Cult activities, I now abhor, detest and unequivocally
denounce all activities pertaining to Secret Cult/Societies and state that I would no longer
participate in any such activities within and outside the institution;
8. THAT if I am caught or implicated in activities pertaining to secret cult and/or the purchase,
sale, possession or consumption of alcohol, hard drugs, cannabis, cigarettes, tobacco or any
form of substance abuse within and outside the University environment, I agree that the
University should impose appropriate sanction(s) as it deems fit in the circumstance;

9. THAT I do not take or consume any hard or illicit drug of any form. That if in the past I have
taken any, I now undertake never to take it again;

10. THAT I undertake to allow the University carry out drug test(s) on me during my stay as a
student of the University AND that the University shall be at liberty to expel me if I am
found/tested positive for any illicit or hard drug.

11. THAT I am not a Homosexual, Gay, Cross-Gender, Lesbian or Transgender. If I am caught or


implicated in any of these acts, I agree that the University should expel me immediately; and

12. THAT I undertake to be of good conduct and to abide by all the regulations in force at Mountain
Top University including the code of dressing, policies and laws of academic and social conduct,
spiritual development and discipline as contained in the Mountain Top University Students’
Handbook, a copy of which has been issued to me on admission into the University.

13. THAT I swear to this affidavit in good faith and conscientiously believing same to be true in
accordance with the Oaths Act of the Federal Republic of Nigeria.
