2024 Skills Testing Rubric Catheter Insertion
Performance Objectives: The student will demonstrate the insertion of a urinary catheter for males
and females.
Grading Criteria:
3 Student fully meets the criteria without assistance
2 Student requires minimum assistance to meet the stated criteria (prompt)
1 Student omits one area of stated criteria, even with prompt
0 Student did not meet the stated criteria
Competency 2.5 will be weighted at 15% (testing assessment) of the total grade in NURS-1100. Within
the 15%, 25% of points will be allocated for steps in planning and 75% of points will be allocated for
implementation.
Resources required
Appropriate urinary catheterization kit—contents vary, but kits typically include the following
sterile items: catheter, drapes, lubricant, cleansing swabs, sterile gloves and a specimen
container.

Supplies for perineal care (face cloths & soap/wash)
Male:
With non-dominant hand, gently grasp the
shaft of the penis below the glans; if the
patient is uncircumcised, retract the foreskin.
Hold the shaft of the penis at a right angle to
the body.
Using uncontaminated dominant hand, cleanse
the meatus with swab sticks, using circular
strokes, beginning at the meatus and working
outward in a spiral motion. Repeat process
three times, using a clean swab stick each time.
Appropriately discarded the cotton balls
without contaminating sterile field.
Pick up catheter and lubricate to a distance of 1-2
inches (this may also be completed prior to
cleansing). Position collection tray so urine will
drain into it.
Ensure non-dominant (contaminated) hand
remains in place throughout procedure.
Inserted the catheter
Female:
Ask the patient to gently push the perineum
down, while slowly inserting the catheter into
the urethral meatus. Advance the catheter
until urine flows through the tubing. After
visual confirmation of urine return into the
tubing, release the patient’s labia, while
maintaining a secure hold of the catheter with
nondominant hand.
Male:
Gently apply upward traction to the penis
while holding it at a 90-degree angle to the
patient’s body. Ask the patient to push down
as if to void, and slowly insert the catheter
through the urethral meatus. Advance the
catheter until urine flows through the tubing.
Stop advancing catheter if there is resistance
or patient reports pain.
Lower the penis and hold the catheter securely
with nondominant hand
Allow the urine to flow into collection tray. When
urine drainage diminishes, withdraw the catheter
slowly and smoothly until it has been withdrawn
completely
Dispose of used supplies and provide perineal
hygiene.
