Pharmacy Sig Code Practice:

Translate the following sig codes used in pharmacy. Be sure to include all parts
(action + quantity + dosage form + route + frequency + duration (if needed))

1. 1 cap po QID x 10 days

2. 1 spr en BID

3. 2 tabs po q4-6h prn

4. Inj 20 u SC TID AC

5. 1 tab po qhs prn

6. Apply AA BID x 5 days

7. 2 gtt OU BID x 7 days

8. 1 tab QOD

9. 1 supp PR q6h prn nausea

10. 1 tab SL q8h prn

11. 1 tsp po TID x 10 days (for a 3 year old child)

14. SS tab po c orange juice qam

Answer Key

1. Take 1 capsule by mouth four times daily for 10 days


2. Use 1 spray in each nostril twice daily
3. Take 2 tablets by mouth every 4 to 6 hours as needed
4. Inject 20 units beneath the skin three times daily before meals
5. Take 1 tablet by mouth every night at bedtime as needed
6. Apply to affected area(s) twice daily for five days
7. Instill 2 drops in both eyes twice daily for seven days
8. Take 1 tablet by mouth every other day
9. Unwrap and insert 1 suppository into the rectum every 6 hours as needed for nausea
10. Place 1 tablet under the tongue every 8 hours as needed
11. Give 1 teaspoonful by mouth three times daily for 10 days
12. Instill 2 drops into the left ear four times daily
13. Apply 1 patch to the skin every 72 hours
14. Take ½ tablet by mouth with orange juice every morning
15. Take 1 capsule by mouth at bedtime as needed
16. Inject 1 mL into the muscle one time
17. Inhale 1 to 2 puffs every 4 to 6 hours as needed
18. Unwrap and insert 1 suppository into the vagina every 12 hours for 7 days
19. Take 1 tablespoonful by mouth every night at bedtime as needed for cough
20. Instill 3 drops into the left eye every 6 hours for 3 days
