lab practice can be divided into three phases

1- pre-analytical phase
2- Analytical phase
3-post-analytical phase
1. Pre-analytical: starts from test request and .
ends at sample processing
In the pre analytical phase, the
important errors are as follows

A. Inappropriate Laboratory Test


Requisition.

* inappropriate laboratory tests vary from


11% to 70% for general .biochemistry
and hematology tests

* 5% to 95% for urine screens and


microbiology

* 17.4% to 55% for cardiac enzymes and


thyroid tests
B-Incomplete Laboratory Request Forms

2. Analytical phase :
begins when sample is prepared in the
laboratory for testing and ends when the test
result is interpreted and verified by the
technologist.
common causes are
1- pipetting errors,
2- equipment malfunction,
3- sample mix-ups,
4- interference (endogenous or exogenous),
5- undetected failure in quality contro
Post-Analytical phase

• Not reported
• Report delayed
• Data Entry error
