1 I have been informed of my rights and duties under the
employment agreements prior to or in the process of engagement
2 I have been advised of any particular conditions applicable to the
job for which I am being engaged and of the particular shipowners’
policies relating to the employment
3 The seafarers’ employment agreement including any collective
bargaining agreement that forms all or part of it, is in accordance
with National Provisions.
4 I have examined my employment agreement before hand to seek
advice before signing, and after it has been signed, I have
received a signed original of the agreement
5 I have been explained and will be familiarized with on-board
complaint procedures, as applicable on the ship.
6 I am not subject to exploitation by the SRPS or by their personnel,
with regard to the offer of engagement on particular ships or by
particular companies
7 I have not faced any kind of pressure or exploitation in my dealings
with Indus
8 The Indus staff was polite to me and I have not paid any expenses
for Recruitment or employment.
9 Company/Seafarers Recruitment Policy was seen and read by me.

10 Quality Policy & Safety and Environment Policy was seen and read
by me.
11 All the certificates Submitted by me for employment are genuine
and checked by me.
12 I have successfully completed Training for Personal Safety on
board ship and have obtained my Indian Seaman book.
