Dear Mr./Ms. Rejuso:

Greetings in Saint Benilde!

The Real Estate Management Practicum Program is in the process of conducting performance
evaluation of its practicum trainees. Students enrolled in the Practicum course obtain part of their course
grade from the performance evaluation of the supervisor. In this regard, I request you to please accomplish
the enclosed performance evaluation form.

In order to facilitate proper accomplishment of the form, kindly follow the rating guidelines:

1. The performance evaluation form has two parts. In Part 1, raters will evaluate trainee(s) assigned
in their unit using grade scale.

2. To ensure objective assessment, the rater is requested to have a copy of the trainee’s job
description in their particular unit/department and all his/her work outputs. These will serve as
references for the performance review.

3. Raters are strongly encouraged to give brief descriptions and/or comments regarding the trainee’s
performance on the spaces provided in Part 2 of the evaluation form.

4. In case the trainee is rotated in several real estate functions under different supervisors, each
supervisor will be requested to complete the performance evaluation form.

5. It is MOST IMPORTANT that the rater directly sends the accomplished evaluation form to me via
email at [email protected].

Should you have any questions or clarifications, please feel free to send me an email. Your participation
in this assessment will surely assist not just the development of the student trainee’s performance in his/her
future work but also in the improvement of the Practicum program of our department. Thank you once
