I. AUTHORITY FOR INVESTIGATION:
1. This investigation was initiated on January 18, 2010, based on the directive
issued by the Chief of Police, Tagbilaran City Police Station, following a complaint filed
by Ms. Maria Santos regarding the negligence of PO1 Juan Dela Cruz in handling her
theft case.
II. DETAILS :
2. Ms. Jane Santos, a resident of Tagbilaran City, reported a theft incident at her
residence to the Tagbilaran City Police Station on January 19, 2010. She claimed that
her jewelry box containing valuable items was stolen while she was away at work.
3. PO1 John Dela Cruz was assigned to investigate the case. Ms. Santos provided
him with a list of missing items and detailed information about the incident. However,
PO1 Dela Cruz did not conduct a thorough investigation, neglecting to collect
fingerprints, interview potential witnesses, or examine the scene of the crime.
4. Ms. Santos followed up with the police station several times, but PO1 Dela Cruz
failed to provide any updates or progress on the investigation.
III. RECOMMENDATIONS :
5. Based on the evidence gathered and the lack of action taken by PO1 John Dela
Cruz, it is recommended that disciplinary measures be taken against him for negligence
in duty.
6. The administrative complaint filed by Ms. Jane Santos should be thoroughly
reviewed, and appropriate actions taken to address the issues raised. It is also
recommended that the Tagbilaran City Police Station implement stricter guidelines and
training programs for officers regarding proper investigation procedures and
communication with victims of crime.

2. To investigate the administrative complaint filed by Mr. Constancio Balane
against PO2 Jimmy Perdegon for misconduct.
Ill. FACTS OF THE CASE:
3. On January 14, 2010, Mr. Constancio Balane, a security guard at Bohol Topics
Resort, reported for duty. He received a bag containing important documents from his
colleague, SG Elias Inguito, which was turned over by a tricycle driver. The bag
belonged to Mrs. Ivy Kong, the owner of a dive shop within the resort. Balane kept the
bag in the guardhouse awaiting the owner's claim.
4. Minutes later, a police officer inquired about the bag, informing Balane that it had
been recorded at Tagbilaran City Police Station. Subsequently, PO2 Jimmy Perdegon,
allegedly the desk officer, arrived and examined the bag. Balane witnessed Perdegon
taking money from the bag, claiming it was a reward from Mrs. Kong for the tricycle
driver. Perdegon offered Balane a share, which Balane initially refused, as he was not
the recipient of the bag from the tricycle driver.
5. Despite Balane's refusal, Perdegon insisted and forced him to accept the money.
Balane reported the incident to his head guard, Mr. Ritgie Felix Verano, who intervened.
Perdegon returned part of the money after Verano's intervention, and Balane inserted it
back into the bag.
IV. DISCUSSIONS:
6. PO2 Jimmy Perdegon's actions demonstrate a breach of trust and misuse of
authority. His decision to take money from the bag and offer a share to Balane indicates
misconduct and abuse of power within the Tagbilaran City Police Station.
7. Balane's refusal to accept the money initially underscores his integrity and
adherence to ethical standards despite pressure from Perdegon. V. CONCLUSIONS:
8. Based on the investigation, PO2 Jimmy Perdegon's actions constitute
misconduct and abuse of authority.
9. The incident warrants disciplinary action against Perdegon and highlights the
importance of upholding integrity and ethical conduct within law enforcement agencies.

I. AUTHORITY FOR INVESTIGATION:
1. This investigation was initiated on January 15, 2010, based on a complaint filed
by Mr. Alucard Smith, a resident of Tagbilaran City, reporting the theft of his laptop
computer from his office at the Tagbilaran City Hall. The investigation is being
conducted by PO1 Maria Rodriguez, assigned to the Criminal Investigation Unit of the
Tagbilaran City Police Station.

III. OPERATIONAL PLAN:
a. Concept of Operation: A convoy of two vehicles will be used. One vehicle will
carry the materials, while the other provides security. A pre-determined route will be
followed. Regular communication will be maintained between the vehicles and
command. b. Tasks: Vehicle preparation, route planning, security detail, material
handling, communication protocols, post-operation debriefing.
c. Operational Support: Two security personnel will accompany the convoy.
Communication equipment will be provided. Emergency response plans will be in
place. d. Security: Armed security personnel will accompany the convoy. The
vehicles will be equipped with GPS tracking and security systems.
e. Plan for Coordination: Regular communication will be maintained between the
convoy and command. A designated communication channel will be used.
Contingency plans will be in place to address unexpected events.

a. Personnel: Two drivers, two security personnel, one mission commander.
b. Logistics: Two vehicles, communication equipment, emergency supplies, fuel.
c. Finance: The budget for the operation is $10,000.

regarding your complaint of assault. However, since you are a minor
under the new Constitution, your consent and guidance are required

Answer: CRISOLOGO DE COLOG Y BARRIO, 34 years old, married, a jeepney


driver residing at No. 7 Sampaguita, Bagong Daan, Bayanan, Muntinlupa
City; and RUDY ANTONIO Y MASLOG, 22 years old, single, a sidewalk
vendor residing at No. 7 Sampaguita, Bagong Daan, Bayanan, Muntinlupa
City.
6. Question: Why are you filing a complaint against CRISOLOGO DE COLOG
Y BARRIO and RUDY ANTONIO Y MASLOG?

Answer: Because they stoned me.

7. Question: When and where did the stoning incident happen?

Answer: Last night, around 11:00 PM, July 21, 2019, at Bagong Daan, Bayanan,
Muntinlupa City.

8. Question: Can you recount the entire incident?

Answer: Last night, while I was walking with my friends from Mang Inasal
Restaurant, on our way home, we passed by Bagong Daan. We were
talking and laughing with each other. Suddenly, someone threw a big
stone at me. A man in a white T-shirt, RUDY CRISOLOGO, grabbed a
stone and threw it at me from behind. CRISOLOGO then let go of the
stone, but then my friends and I were punched.
9. Question: What happened next?

Answer: My friends took me to the Barangay hall. Then, an ambulance came and
took me to OSMUN (Muntinlupa City Hospital).

10. Question: Is there anything else you want to add?

Answer:. No, sir.

11. Question: Are you willing to sign this as proof of your statement?
