Process Safety Fundamentals (PSF) Questionnaire

The objective of this questionnaire is to collect information on the main issues that can prevent compliance with process safety fundamentals, giving hints to the
management about how address the issues.
If any question does not apply to your job/duties, please mark it as N.A. (not applicable).

Objective:
