Physical Assessnent Script
Assessment serves several essential purposes in client care. First, it allows us to obtain baseline data
about the client's functional abilities. Second, assessment helps to supplement, confirm, or refute data
obtained in the nursing history. Third, it is crucial for obtaining data that will help establish nursing
diagnoses and plan of care. Finally, assessment allows us to evaluate the physiologic outcomes of
health care and the progress of a client's health problem.
In conducting a thorough physical examination, there are four primary methods we use to gather
essential information. First, we have Inspection. This is simply a visual examination using our sense of
sight. Next is Palpation. This involves using our hands to feel different areas of the body. Third, we use
Percussion. This method involves tapping the body lightly with our fingertips. Finally, there’s
Auscultation. This method is the process of listening to sounds produced within the body.
Today, I’ll be introducing the essential equipment we use in a physical assessment. First, we have a
wristwatch with a second hand. Next, we use a thermometer to check body temperature. The
sphygmomanometer, used for measuring blood pressure. A stethoscope is essential in listening to heart,
lung, and bowel sounds. For ear examinations, we use an otoscope. Then, there’s the Snellen chart. A
penlight is used to check the client’s pupils. We also use a safety pin. A percussion hammer helps assess
reflexes. A tape measure is useful for measuring body parts. The tongue depressor. Gloves are a must
for hygiene and safety. A cotton applicator can be used for applying medication. A millimeter or
centimeter ruler. The gauze square used for cleaning. A pencil is handy for taking notes. Wisps of
cotton. Lastly, test tubes of hot and cold water (optional). Each of these items plays a specific role in the
physical examination process.
Here are the steps to prepare for a physical examination. First, wash your hands to reduce the
transmission of microorganisms. Next, prepare all the materials needed for the examination. Now,
prepare the environment. Make sure the room temperature is comfortable. Then, explain the
procedure to the client. Take a moment to clearly explain what will happen. Finally, provide for the
client’s comfort by allowing them to empty their bladder or bowel if necessary.
