RE EVALUATION OF THE SHORT-LISTING (SL) DOCUMENTS
PROCUREMENT OF
SELECTION OF EMERGENCY PREPAREDNESS CONSULTANT (EMPC) STRENGTHENING OF NATIONAL
REFERRAL HOSPITALS AND VERTICAL TECHNICAL UNIT PROJECT (ISDB) FINANCING IDN 1031 IN INDONESIA

I. On this day, Friday, dated on Eighteen, month October, year Two Thousand Twenty-Four, the
selection working group re-evaluated documents related to the IsDB reportat Procurement Bureau of
The Ministry of Health meeting room, Jalan H.R. Rasuna Said Blok X5 Kav. 4-9, Jakarta the meeting
for evaluation of shortlisting documents of the Selection of Emergency Preparedness Consultant
(EMPC) Strengthening of National Referral Hospitals and Vertical Technical Unit Project (IsDB)
Financing IDN 1031, Directorate General of Health Service, Ministry of Health has been held.

II. The meeting was started at 02:01 p.m. local time on 18th October, 2024. Evaluation meetings were
held several times from 18th October, 2024 led by Edi Susiyanto as the Chairman of Selection
Committee for Selection of Emergency Preparedness Consultant (EMPC) Strengthening of
National Referral Hospitals and Vertical Technical Unit Project (IsDB) Financing IDN 1031,
Directorate General of Health Service, Ministry of Health, Fiscal Year 2024.

III. Attending parties in the meeting were as follow:

Fiscal Year 2024.
V. Scope of Evaluation:
Scope of Evaluation of short-listing document for Selection of Emergency Preparedness
Consultant (EMPC) Strengthening of National Referral Hospitals and Vertical Technical Unit
Project (IsDB) Financing IDN 1031 conducted by the selection committee was presented as
below:

1. Type of association: Single or Joint Venture (JV Lead/JV Partner)

2. Legal Administration:
a. Valid businees entity certificate
b. Business Identification Number (NIB);
c. Legal firm establishment and its amendments;
d. Tax Identification Number (NPWP)
e. Statement The firm legally is not in court/legal issues, not bankrupt, and have
capacity to assigning contract in accordance with law and regulation;
f. Firm should have a good reputation and not be included on the black list by
government institution;
g. declaration that all documents and information required are complete, true and correct

3. Firm Experience:
a. Core business in consultancy and minimum 10 years in business.
b. Relevance similar experience of at least 1 (one) similar assignment in the last 10 (ten)
years in the field, which shall include but not be limited to experience in conducting
capacity building/ skill development/ human skill upgrading/ training, preferably funded
by MDBs or other international funding institution. Information to be provided should
include name of assignment, name and full contact address of the client, contract
value (in equivalent US dollars) and period (dates) of execution of assignment and the
role played in the assignment (Lead or JV partner).
c. Not have been awarded a contract as a consultant to the Client related to the project
of the Strengthening of National Referral Hospital and Vertical Technical Units Project.

VI. Result of the Re-evaluation:

The result re-evaluation of short-listing document for Selection of Emergency Preparedness
