I am Jaydelin Toledo , the mother of Arjolyn Toledo a student from BSED English
2A. I am humbly writing to inform you about my child's recent health condition and
to kindly request your understanding regarding her missed classes and activities since
she was absent during your class last time.

Arjolyn Toledo was recently diagnosed with a urinary tract infection (UTI), which
caused her to experience severe symptoms such as persistent lower abdominal pain,
headaches, chills occurring every three hours, and overall physical weakness. Despite
her strong determination to attend her classes and complete her academic
responsibilities, her condition prevented her from doing so.

After a few days of rest and medical care under her doctor’s guidance, she has now
recovered and is ready to return to school. She also has the medical certificate and all
relevant documentation from the doctor as proof of her illness, which she is prepared
to present. We kindly ask for your compassion and consideration in allowing her to
make up for her missed activities and assignments. As an aspiring teacher, she values
her education deeply and is eager to prove her dedication by catching up on any
academic requirements she may have missed during her absence.

We sincerely apologize for any inconvenience this may have caused. Your
understanding and support in this matter would mean a great deal to her and to our
family. If there are specific instructions or deadlines for making up her missed tasks,
please do let her know, and she will do her utmost to comply promptly. Thank you
