IMPORTANT POINTS:

 Assessor must reach on Centre location on or before 9:30 AM without any


fail. Assessment timing (10:00 am to 6:00 pm). All candidates preferably
should be present 15 minutes prior to the schedule time of assessment.
Assessment location must be same as per the SIP TC address.
 Assessor must ensure all prescribed evidence are collected.
 The assessor should mark attendance for NON-PMKVY batches on the
NON-PMKVY assessor app on the same day of assessment.
 The attendance should be mark on Aadhar enabled PMKVY assessor app
for all PMKVY batches on the same day of assessment.
 Non-Assessor App functionality should not be used for any batch under
PMKVY 4.0.
 Assessors to update their Aadhar card and fingerprint before the day of
assessment (Only if they have not updated) and check the profile should
be Aadhar verified.
 Practical exam can be conducted in a group of 2-3 candidates and viva
must be conducted individually for all the candidates. Candidates will
speak their name in the practical/viva videos and all videos should be
clear and audible with the minimum duration of 3 minutes.
 Assessor must give orientation to all candidates about assessment process
before starting the exam including the timings, language of exam, passing
marks etc.
 Assessor may ask TP to download the biometric attendance online on the
day of assessment as part of the verification process.
 It is mandatory for the candidates to present at least one original Govt. ID
proof (hard copy) on the day of assessment. Xerox or colored print outs of
Aadhar or other Govt. ID proof will not be considered. If AA and assessor
found any invalid Aadhar or any Govt. ID proof on the day of assessment
than AA and assessor can verify those candidates by downloading e-
Aadhar of that candidate.
 Refrain from using the whitener on the attendance sheet
 The absent/dropout candidates should be marked with the red pen on the
attendance sheet.
 Assessor should able to justify/explain for the performance/result of the
candidates.
 assessor should verify the availability of assessment tools and equipment
in the training Centre.
 In each batch, the practical videos should able to showcase the followings
- {1. Working on the tools and equipment 2. Outcome (result) of the
practical task} in which it should be visible whether the objective of that
job role has been fulfilled; based on which the assessor will mark the
competency of the candidate for the job role.

Evidence to be shared

Documents

a. Attendance sheet.
b. result sheet
c. Biometric training attendance.
d. Front and back scanned copy of Aadhar card (Aadhar card must be same (or
e-Aadhar) as presented during the authentication of candidate.
e. TP Feedback Form
f. Annexure-M (Assessor Feedback Form)
g. Annexure-N (Candidate Feedback Form) – For Minimum 5 Candidates
h. Code of Conduct
i. Covid Declaration
j. UPSDM Annexure (Signed and stamped by DPMU on the day of assessment) –
Only for UPSDM batches
k. All present candidate’s enrollment forms with their passport size photograph
attached on it.
l. Tools/Equipment list as per SIP (Duly cross verified by assessor and TP)

Images
i. Assessor selfie in front of the Centre with branding [2 Images (In and Out)].
ii. Centre picture (Front view, classroom & lab).
iii. Tools/Equipment pictures (Should be visible and countable as mentioned on
documents).
iv. Trainer ID & TOT certification.
v. Practical Assessment (3 different pictures).
vi. Theory Assessment (3 different pictures).
vii. Viva Assessment (3 different pictures).
viii. Group pictures of all present candidates (Candidate head count should be
the same as mentioned on the attendance sheet) with certified trainer and
assessor. The trainer and assessor will carry name tag during capturing the
group picture.
ix. Each candidate with Aadhaar pictures (Must be clear) and all alternative id
proofs (If any).
x. Biometric attendance screenshots from the assessor app (For all candidates) –
For all PMKVY batches.

Videos
i. Practical Assessment (2 different Videos) - Minimum 3 minutes.
ii. Theory Assessment (2 different Videos) - Minimum 3 minutes.
iii. Viva Assessment (2 different Videos) - Minimum 2 minutes.
iv. Assessment Orientation (1 Video) - Minimum 3 minutes.
v. Infrastructure and lab (1 Video)
vi. E-Aadhar downloading videos in front of assessor (If any).

All pictures and videos must be with geo tag and date & time stamped.
Candidate’s Aadhar holding picture must be without mask and Aadhar should be
readable and image should be visible.
The assessor must capture biometric authentication screenshots for all present
candidates from the assessor app while authentication of the candidates
