with the Department of Clinical Pharmacy, humbly request your kind approval for our research project
as a prerequisite for the award of a certificate of Doctor of Pharmacy Degree (PharmD ) by the
University of Jos.

The proposed topic is "Prevalence and predictors of herbal medicine use among type 2 diabetes
patients in Jos Metropolis " A cross-sectional survey in Tertiary Health Facilities Kindly Find attached
