GARBHA SANSKAR AWARD FORM

Applications are invited from all families who have practised the Santulan Garbha
Sanskar as outlined in Shreeguru Dr. Balaji Tambe’s book.

This year, the 11th Garbha Sanskar Puraskar will accept applications from children
born between 1 Jan, 2019 to 30 June, 2021 (both dates inclusive). Please ensure to
fill in all the fields in the form below as we will review all the details in your
application for the award.

● If you have checked and observed some developmental differences in your
child, then such progress reports should be attached with the form. Similarly,
please attach proof of the progress of your child; like photos, videos, reports,
etc.You may put these on a CD/DVD/Pen Drive and send it to us along with
your application.

The application will not be considered in case the information is found to be


incomplete. Please ensure at least 3 print quality photos of the child are included in
the application. Last date for application - 22 June, 2022.

1. Post it in a sealed envelope with all the above requirements to
Garbha Sanskar Puraskar, Atmasantulana Village, Near MTDC Holiday Resort,
Karla 410 405 Maharashtra. (Mobile: 9689926002)
2. Drop the application along with the above requirements at any of the 6
Santulan centres at Karla, Mumbai, Pune, Nasik, Kolhapur or Ahmedabad.
