Preparation

1. Understand why the client is taking the medication, including


classification, contraindications, dosage, side effects, and nursing
considerations.
2. Check the MAR for drug details (name, dosage, frequency, route,
expiration). Resolve discrepancies with the prescriber’s latest order
3. Verify the client’s ability to swallow and any restrictions (e.g., NPO,
vomiting).
4. Organize supplies and the MAR for one client at a time.

Procedures

5. Perform hand hygiene and follow infection protocols.

6. Unlock the medication system.

7. Select, verify, and check the medication’s expiration against the MAR.

8. Calculate and prepare the correct dose, double-checking with the MAR.

9. Place unit-dose tablets in a medicine cup, avoid touching them, and


separate narcotics or special-assessment drugs.

10. Break scored tablets if needed, follow policy for unused portions, and
crush only when allowed.

Liquid Medication

1. Mix the medication thoroughly before pouring and discard if discolored


or cloudy.
2. Place the cap upside down to avoid contamination.
3. Hold the bottle with the label next to your palm and pour away from
the label.
4. Fill the medication cup at eye level, aligning the bottom of the
meniscus with the measurement line.
5. Wipe the bottle lip with a paper towel before capping.
6. For small amounts (less than 5 mL), use a sterile syringe and label it
with the medication name and route (PO).
7. Keep unit-dose liquids sealed and open them at the bedside.
All Medications

1. Place the prepared medication and MAR together on the cart.


2. Recheck the label before returning the container to storage.
3. Do not leave prepared medications unattended.
4. Lock the cart before entering the client’s room.
5. Verify the room number with the MAR if policy restricts MAR removal.
6. Ensure client privacy.
7. Prepare the client: introduce yourself, verify identity, and assist to a
sitting or side-lying position.
Purpose:
To deliver medication by mouth for therapeutic effects.
8. Explain the medication’s purpose clearly.
9. Administer at the correct time following agency guidelines.
10. Provide water or juice for swallowing; check for incompatibilities.
Assist as needed, giving one pill at a time. Address taste issues with ice
chips or compatible food.
11. Stay until all medications are swallowed.
12. Document the medication, dosage, time, client’s response, and
your signature. Record any refusals or omissions.
13. Dispose of supplies, replenish stock, and return the cart.
