April 21. Used device for 20 minutes. Took the device off. He ‘reports that he feels
more balanced and stable. Toes are still numb. A little bit of tremor in my left hand.

April 22. After the device, toes are still numb but not hurting, feeling
grounded/swinging/steady. Usually there is unsteadiness when standing. More steady t
unsteady now.

April 26. His stopping and turning when walking is a lot more improved.

The progression of the disease has declined dramatically. The patient exhibited improved recognition
compared to baseline. We also observed reduced cognitive decline. Her eating, sleeping and bathroom
use is good. Her appetite is great. Her mood significantly improved. She doesn’t get angry like before at
all.
