Read the following case carefully and on the basis of your understanding about the disorder do

the analysis of the case.


Case Analysis will include: Symptoms, Duration (if mentioned), Identification of Disorder,
Cause associated with disorder and Treatment.

1. Narayan is a 20-year-old male who is in his second year of college. He is seeking


treatment due to persistent fears that campus security and the local police are tracking
and shrivelling him. He cites occasional lags in his internet speed as evidence that
surveillance devices are interfering with his electronics. His intense anxiety about this
has begun getting in the way of his ability to complete schoolwork, and his friends are
concerned – he says they have told him, “you’re not making sense.” Narayan
occasionally laughs abruptly and inappropriately and sometimes stops speaking mid-
sentence, looking off in the distance as though he sees or hears something. He expresses
concern about electronics in the room (phone, computer) potentially being monitored
and asks repeatedly about patient confidentiality, stating that he wants to be sure the
police won’t be informed about his treatment. His beliefs are fixed, and if they are
challenged, his tone becomes hostile.

2. Dev is a 28-year-old man who has struggled with alcohol use for most of his teenage
and young adult life. He began drinking in high school, at age 15, and dropped out of
college partway through his first year because he was missing so many classes due to
alcohol use. Whenever he tries to stop drinking, he gets nauseous, shaky, and
disoriented. Diagnosed with alcohol use disorder his doctors is also looking forward for
comorbid of other mental illness in him as within the last 4 months, he has withdrawn
from his friends, feels sad almost every day, and has frequent suicidal ideation. His
girlfriend also left him because of his addiction to alcohol. His appetite has decreased,
and he has lost interest in his day today activity. He has frequent arguments with his
parents, who are now threatening to cut him off financially because he spends most of
his money on alcohol, his attempt to stop intake of alcohol has failed and he reports
feelings of guilt and worthlessness over being unable to stop drinking.
3. A 35-year-old man loses his beloved uncle suddenly to an accident. Two years later,
he developed an obsession that harm would result to loved ones if he did not move or
walk in a special way. He knew the idea was strange and silly, but he could not stop
thinking about it. The man developed elaborate compulsions that involved stepping in
a just right way. The process became time consuming and cumbersome. Going out in
public by himself or with family became an ordeal

4. Ms x is a 21 year old undergraduate studying Accountancy, who has been distressed


for the past few weeks by some disturbing thoughts. She has thoughts of slapping her
friends (for no reason) when they study together and also has mental images of them
indulging in sexual acts. She finds these thoughts and images to be repulsive and
disturbing, but they continued to be intrusive and difficult to resist. She is even more
distressed when she goes for her regular prayers: she will have sudden impulses to
blaspheme (the act of insulting) the name of God. She is afraid that she might lose
control one day and shout blasphemies in public.

5. Mrs X is a 28 year-old married female. She has a very demanding, high stress job as a
second year medical resident in a large hospital. Mrs X has always been a high achiever.
She graduated with top honors in both college and medical school. She has very high
standards for herself and can be very self-critical when she fails to meet them. Lately,
she has struggled with significant feelings of worthlessness and shame due to her
inability to perform as well as she always has in the past.For the past few weeks Mrs X
has felt unusually fatigued and found it increasingly difficult to concentrate at work.
Her co-workers have noticed that she is often irritable and withdrawn, which is quite
different from her typically upbeat and friendly disposition. She has called in sick on
several occasions, which is completely unlike her. On those days she stays in bed all
day, watching TV or sleeping.At home, Mrs X’s husband has noticed changes as well.
She’s shown little interest in sex and has had difficulties falling asleep at night. Her
insomnia has been keeping him awake as she tosses and turns for an hour or two after
they go to bed. He’s overheard her having frequent tearful phone conversations with
her closest friend, which have him worried. When he tries to get her to open up about
what’s bothering her, she pushes him away with an abrupt “everything’s fine”.
Although she hasn’t ever considered suicide, Mrs X has found herself increasingly
dissatisfied with her life. She’s been having frequent thoughts of wishing she was dead.
She gets frustrated with herself because she feels like she has every reason to be happy,
yet can’t seem to shake the sense of doom and gloom that has been clouding each day
as of late.

6. A 21 year old college freshman is brought to the emergency room by his roommate
for not leaving his dormitory room for 2 weeks. The roommate reports that the patient
has not left his dormitory room for 2 weeks and his room is in disarray. He describes
the patient as being “normal” until about 3-4 months ago. He states that he noticed
that the patient stopped going to social activities and spent most of his time in his
room. He also states that the patient sometimes makes odd comments. He has stopped
going to his classes and his grades have been declining. He also states that for about
a week the patient has stopped eating and drinks only canned beverages and insists
on keeping the shades down on the windows and has said that he is worried that
someone is watching him.

7. Mr X is a 21 year-old business major at a large university. Over the past few weeks
his family and friends have noticed increasingly bizarre behaviors. On many
occasions they’ve overheard him whispering in an agitated voice, even though there
is no one nearby. Lately, he has refused to answer or make calls on his cell phone,
claiming that if he does it will activate a deadly chip that was implanted in his brain
by evil aliens. His parents have tried to get him to go with them to a psychiatrist for
an evaluation, but he refuses. He has accused them on several occasions of conspiring
with the aliens to have him killed so they can remove his brain and put it inside one
of their own. He has stopped attended classes altogether. He is now so far behind in
his coursework that he will fail if something doesn’t change very soon. Although Mr
X occasionally has a few beers with his friends, he’s never been known to abuse
alcohol or use drugs. He does, however, have an estranged aunt who has been in and
out of psychiatric hospitals over the years due to erratic and bizarre behavior.
