Re: Yifang Lu - Personal Support Worker Employment Verification

Please be advised the above-named was an employee of Carefor Health & Community
Services from April 29, 2019 to August 11, 2019 as a Personal Support Worker. Yifang worked
a total of 384.69 hours with Carefor.

Yifang’s base salary was $15.00/hour; paid on a bi-weekly basis. Yifang was a casual employee
and worked an average of 48.08 hours bi-weekly. Yifang was permitted to work a maximum
of 44 hours per week and she also earned 2.1% in lieu of benefits and 4% vacation on top of
her base salary.

Yifang earned an extra $1.00 per hour when she works in the evenings and/or on weekends
and currently received an extra $4.00 per hour from the Wage Enhancement grant provided
by the Government for all PSWs that have clients associated with the CCAC. In addition, Yifang
was paid $.047 per kilometer for her travel time as well.

Her role as Personal Support Worker is to provide personal care, medication assistance and
supports clients in a community health care setting, both in personal homes and retirement

POSITION TITLE: Personal Support Worker


REPORTS TO: Supervisor, Personal Support Services
AUTHORIZED BY: Director, Ottawa Community Support Services

POSITION OVERVIEW

The Personal Support Worker provides support and care to individuals and families in their
home and in retirement residences. The Personal Support worker provides personal care, light
housekeeping, delegated exercise programs and activities of daily living (ADLs) as prescribed
by each clients individual care plan.

The Personal Support Worker collaborates with all members of the health care team to meet the
needs of individuals attain their optimal level of independence and well being.

KEY DUTIES AND RESPONSIBILITIES

• Provides support as indicated on the care plan, with tasks such as; bathing, mouth and skin
care, dressing and grooming, assisting in walking and transfers, bedside care, etc.
• Performs delegated tasks under the direction of the Supervisor, Personal Support Services
• Provides laundering of clothes and linens
• Provides light cleaning of household
• Provides respite care
• Provides meal set-up and preparation
• Consults with the Client Services Coordinator and/or Supervisor, Personal Support Services
regarding changes in the client’s or the family’s personal needs or any potential need for
change in care plan
• Documents specific care provided to client, any changes to client’s condition or concerns
with client’s home environment. Documentation is completed in the Mobility app
• Acts in compliance with the Occupational Health and Safety legislation, regulations, and
Carefor policies and procedures
• Other related duties/tasks as assigned by the Supervisor, Personal Support Services
regarding

SUPERVISORY RESPONSIBILITIES

EDUCATION, EXPERIENCE AND SKILLS REQUIRED

• Certificate in Home Support Worker (Level II, III), Personal Support Worker or Developmental
Service Worker program, Health Care Aide, Foreign Trained BScN or doctor, Paramedic
Graduate, BScN Nursing Student, or second year RPN student.
• National Academy of Careers Colleges Certificate, if applicable
• Household management experience
• Experience working with elderly or ill individuals or children
• Ability to manage confidential information
• Ability to work independently
• The ability to communicate effectively in English, both written and verbal

OTHER QUALIFICATIONS

• Vulnerable Sector Check


• Current Standard First Aid Certificate, an asset
• Current CPR level C Certificate, an asset
• Food Handlers Certificate, an asset
• Non-Violent Crisis Intervention Certificate, an asset
• Up to date immunizations for diphtheria, tetanus, polio, rubella and hepatitis
• Negative tuberculosis test

WORKING CONDITIONS

• Work in client homes and retirement homes


• Work requires regular driving or use of public transportation, including travel in inclement
weather
• Work requires incumbent to bend, sit, stand, kneel for short periods of time
• Work in different social and economic environments
