1) Write a summary of findings of Jackie's Modified Fatigue Impact Scale (MFIS) Results.

The MFIS is part of the MS Quality of Life Inventory, each scale can be scored and

interpreted independently. This summary measures and describes how MS fatigue is

showing up in Jackie's life.

The Modified Fatigue Impact Scale (MFIS) was administered to assess how fatigue

impacts the client's physical, cognitive, and psychosocial functioning. Client scored 42 out of 84,

with higher scores indicating greater fatigue. The score of 42 indicates a moderate level of

fatigue. Client experiences difficulty with home management, meal preparation, childcare, and

maintaining bedtime routine with husband due to fatigue from managing daughters' schedules,

work demands, and personal expectations. This limits the client’s ability to stay organized,

complete tasks efficiently, and engage meaningfully with family. OT services recommended to

increase the level of independence and occupational performance in home and community

settings.

2) Write a summary of findings of Jackie's OT initial assessment results (use the Case

information and video for the synthesis of results to include in your summary). Your

summary must justify the need for OT services & include a recommendation for frequency,

intensity, duration of OT services that will be provided. DO NOT INCLUDE SPECIFIC

INTERVENTIONS - THIS IS NOT A SESSION PLAN. Bonus - include 1 LTG & STG

that should align with your recommendations.

Client was referred to OT services due to fatigue and coordination challenges associated

with relapsing-remitting multiple sclerosis (RRMS). PMH is significant for optic neuritis and

long-term management of RRMS with medication. Client is a 38-year-old woman living in a


3-bedroom/2-bathroom ranch-style home with her husband and two daughters. The house has a

front entrance with four steps and one railing, as well as a side entrance with a mudroom and

stairs leading to the main living level. Client enjoys spending time with her family, participating

in her daughters' extracurricular activities, and hosting gatherings with friends. Prior to HPI,

client was independent in ADLs and managed household and children's schedules with minimal

difficulty. Currently, fatigue and impaired coordination limit client’s ability to complete

household tasks, childcare responsibilities, and maintain organization. Client would benefit from

OT services 60 minutes, 3 times per week for 6 weeks to increase level of independence and

occupational performance in home and community settings.

STG: Client will identify and implement at least two energy conservation techniques during

meal preparation without exceeding a fatigue level of 5/10 by 2 weeks.

LTG: Client will independently complete home management tasks using energy conservation

strategies to reduce fatigue by 6 weeks.
