E. The Applicant-Competency Assessor Collected evidence to make Assessment Decision :

INDICATORS:
E1: Evidence Gathering. The Applicant-Competency Assessor:
 Ensured that the candidate understood and performed all tasks provided in the Specific Instruction for
the Candidate
 Implemented the evidence gathering process as prescribed in the Assessors Guide.
 Observe the candidates during Assessment and recorded observations on the Rating Sheets.
 Recorded on individual Rating Sheet the details of evidence collected while observing the candidate
during the assessment process.
 Use the Rating Sheet extensively to record findings / observations/performance of the candidate
 Used all the Assessment Methods identified in the Evidence Plan.
F. Assessment documentation reporting.
The Applicant-Competency Assessor:

INDICATORS:
 Request the candidate to sign the Rating Sheet and the CARS at the end of the feedback session.
 Recorded assessment outcomes promptly and accurately on the prescribed forms.
 Ensured that the contents/data and result in the Candidate’s copy of the CARS are the same as in the
TESDA’s copy.
 Reviewed the result in the RWAC viz. result indicated in the assessment documents, i.e Rating Sheet,
CARS, Attendance Sheet before affixing signature.
 Returned to the TESDA Rep/AC Manager copy of the Assessor’s Guide/Specific Instructions for the
Candidate/Rating Sheets/Attendance Sheet/CARS/RWAC promptly.
 Maintained the integrity and confidentiality of the Assessment outcome.

ASSESSOR GUIDE must:


1. PUNCTUAL, PROPER DRESS & GROOM, I.D
2. CHECK THE ASSESSMENT AREA IF CONDUSIVE (TOOLS, EQUIPMENT,MATERIALS)

4. Today I receive 10 copies of Attendance Sheet, CARS, Rating Sheets & Specific Instruction for the candidates & 1 copy of the
Assessor’s Guide. I have also here your Accomplished SELF ASSESSMENT GUIDE, this is the indication that you know and can
perform all the units of competencies for this qualification.
5. GREETINGS! (to the candidates, Welcome to BAYBAYTECH your Accr.Assessment Center for PLM I)

7. ACKNOWLEDGEMENT (AC MANAGER, TESDA REPRESENTATIVE From TESDA P.O) You don’t have to worry about their presence,
because they are just here to oversee, if the conduct of Assessment Methodologies & procedure. They will not assess you.
8. ATTENDANCE: ( I will now check your attendance, so as I call your name please come forward and present to me your
ADMISSION SLIP and pls. sign the attendance) ( let the assesse sign the attendance.)
9. CONDUCT ORIENTATION & EXPLAIN CONTEXT & PURPOSE of ASSESSMENT of ASSESSMENT EVIDENCE TO BE COLLECTED
 (The purpose of this assessment is to measure your ATTITUDES, KNOWLEDGE & SKILLS as well as to determine the
level of your competency. And I will collecting evidences through DEMONSTRATION & ORAL QUESTIONING and this
will help me ensure your underpinning knowledge and skills on PLM I)
 The Units of Competency to be Assesses are the BASIC, COMMON, & CORE COMPETENCIES. But we will just be
assessing CORE because the basic and Common are already embedded with the CORE.
10. ASSESSMENT ARE BASED ON 4 DIMENSIONS OF COMPETENCY
1. TASK SKILLS – the ability to perform individual Task
2. TASK MANAGEMENT SKILLS – The ability to manage a number of different task or Activities within the job role / environment.
3. CONTINGENCY MANAGEMENT SKILLS – the ability to respond to irregularities and breakdowns in routine
4. JOB/ROLE MANAGEMENT SKILLS – the ability to deal with responsibilities and expectations of the workplace, including
working with others.

longer allowed. Using Cellphone is not allowed during the assessment, If you have personal needs, please ask my permission so
that we can address your need immediately.)

16. OHS (AS we go along with our Assessment please Observe Safety Practices Also when accidents Happen or occur please call my
Attention immediately so I can give you First Aid, FIRST AID KIT .)
- In doing the activity Kindly wear PPE, observe health and Sanitary practices.
17. ROLE OF ASSESSOR ( Me as an assessor my role is just to observe and write down what I have observe during the assessment
BUT as an assessor I have the right to stop the assessment if I observe any untoward behavior, especially if it can harm the other
candidates.)
18. RIGHT TO APPEAL SYSTEM & REASSESSMENT PROCEDURE
- And you have found to be NOT YET COMPETENT, don’t worry about it, because you can be re assess anytime at your
convenience.
-- If you have doubts or questions with the assessment result, you have the right to appeal. Just submit a letter to the TESDA
Provincial Office where the center and the assessor are accredited.

19. ISSUANCE OF NC CERTIFICATE


IF COMPETENT:
- E- CERTIFICATE AVAILABLE 24 HOURS AFTER THE COMPETENCY ASSESSMENT
- MAKE SURE YOUR EMAIL IS ACTIVE AND YOU CAN OPEN IT
- VALID FOR 5YEARS
- RENEW 1 MONTH BEFORE EXPIRE
- 50 PESOS FEE FOR RENEWAL
- PASSPORT SIZE ID
20. CONFIDENTIALITY OF ASSESSMENT OUTCOMES
- With the result of Assessment, we observe confidentiality, this is to ensure your integrity, these results is only
made available to the right personnel and to the institution were you had your actual training. Lastly may I remind
everyone that please refrains giving any information outside regarding the process of the assessment?
21. PROVIDE SPECIFIC INSTRUCTION
A: So now, I will distribute the specific instruction. Please read and understand it carefully. If you have difficulty, I
am willing to translate it in our local dialect if you want.

measurements.
- Please inform that after this demonstration we will be having an oral questioning.
- COLLECT THE EVIDENCE AND MAKE ASSESSMENT DECISION *check the papers while observing.*
