E1: Evidence Gathering. The Applicant-Competency Assessor:
Ensured that the candidate understood and performed all tasks provided in the Specific Instruction for
the Candidate
Implemented the evidence gathering process as prescribed in the Assessors Guide.
Observe the candidates during Assessment and recorded observations on the Rating Sheets.
Recorded on individual Rating Sheet the details of evidence collected while observing the candidate
during the assessment process.
Use the Rating Sheet extensively to record findings / observations/performance of the candidate
Used all the Assessment Methods identified in the Evidence Plan.
F. Assessment documentation reporting.
The Applicant-Competency Assessor:
INDICATORS:
Request the candidate to sign the Rating Sheet and the CARS at the end of the feedback session.
Recorded assessment outcomes promptly and accurately on the prescribed forms.
Ensured that the contents/data and result in the Candidate’s copy of the CARS are the same as in the
TESDA’s copy.
Reviewed the result in the RWAC viz. result indicated in the assessment documents, i.e Rating Sheet,
CARS, Attendance Sheet before affixing signature.
Returned to the TESDA Rep/AC Manager copy of the Assessor’s Guide/Specific Instructions for the
Candidate/Rating Sheets/Attendance Sheet/CARS/RWAC promptly.
Maintained the integrity and confidentiality of the Assessment outcome.

Attention immediately so I can give you First Aid, FIRST AID KIT .)
- In doing the activity Kindly wear PPE, observe health and Sanitary practices.
17. ROLE OF ASSESSOR ( Me as an assessor my role is just to observe and write down what I have observe during the assessment
BUT as an assessor I have the right to stop the assessment if I observe any untoward behavior, especially if it can harm the other
candidates.)
18. RIGHT TO APPEAL SYSTEM & REASSESSMENT PROCEDURE
- And you have found to be NOT YET COMPETENT, don’t worry about it, because you can be re assess anytime at your
convenience.
-- If you have doubts or questions with the assessment result, you have the right to appeal. Just submit a letter to the TESDA
Provincial Office where the center and the assessor are accredited.
