Filling CCF – One column at a time
❖ Col 1 : Tick the type of benefit you are applying for. If applying for PF, Pension
and EDLI tick all three. Please note that claim will be processed only for the
benefit which has been claimed/ ticked.

❖ Col 2 : Enter NAME of the deceased member in CAPITAL LETTERS

❖ Col 3 : Furnish Father’s name and Spouse name of deceased member (Ensure
that it is matching with data available with EPFO. If not matching, Joint
Declaration Form signed by claimant and employer along with documentary
proof may be submitted for correcting the same)

❖ Col 4 : Enter Marital status of deceased member – Married or Unmarried

❖ Col 5 : Mention AADHAR number, UAN and PF account number of deceased


member (Please note that UAN and PF Account No are different)

❖ Col 6: Enter Date of Leaving Service. This should be as per employer’s records –
i.e the date up to which the member continued to be on the rolls of the estt.

❖ Col 7 : Details regarding Scheme Certificate (i.e. details of previous EPS


(Pension) service), if available – Whether issued or not, SC number and the
concerned EPFO office from where it has been issued

❖ Col 8 : Period of Non-contributory Period (NCP days -TO BE FILLED BY


EMPLOYER) – If multiple periods are there, separate list may be enclosed.
(Note : NCP is days on which no salary was earned)

❖ Col 9 : Date of Death of the member – Should be matching with Date in Death
Certificate.

❖ Col 10 - Whether the member had died while in service (Yes / No) – i.e while in
employment or not. VERY IMPORTANT. IT WILL BE SUBJECT TO PROPER
VERIFICATION BY EPFO OFFICE. It should be matching with employer’s
records. Hence claimant/ employer to fill it diligently.

❖ Col 11 : Enter all required details of ALL ELIGIBLE CLAIMANTS (as in SFC) in the
table provided. NO COLUMN SHOULD BE LEFT BLANK.

❖ Col 12 : Enter BANK DETAILS of all claimants for PF & EDLI. For PF and EDLI, any
Scheduled Bank (including Co-operative Banks) is allowed. NO COLUMN
SHOULD BE LEFT BLANK. The name in enclosed cheque leaf/ pass-book should
be matching with that mentioned in claim.

❖ Col 13 - Enter BANK DETAILS of PENSION beneficiaries only. Please note the
following while filling this column :
➢ All pension beneficiaries should be having accounts in the same bank and branch.
➢ Only following banks are accepted for pension disbursement in Bangalore as on
August 2021 - SBI, Canara Bank, ICICI Bank, HDFC Bank, Axis Bank. The claimant for
pension has to mandatorily open an account in any of the above banks if not already
having one in these banks.

➢ If the pension disbursing bank branch is outside Bangalore, get the list of
approved banks for the particular region at:

❖ Col 14 : Enter complete postal address of the claimant with Pincode.

Don’t Miss These!
✓ ALL CLAIMANTS to mention NAME & put SIGNATURE.

✓ Employer’s (Authorised Signatory) name & signature along with seal of company.

✓ Primary Claimant to mention MOBILE NUMBER in top right column of CCF

✓ For dependent parent pension in case of unmarried member’s death and if one
of the parents had also expired, the Death Certificate in respect of the deceased
parent is also required to be submitted

✓ Employer to ensure that Date of Joining and Date of Leaving/ Death has been
updated and correctly available in EPFO database.

Please Note : A nomination filed before marriage becomes invalid once he/ she gets
married. A fresh nomination was required to be filed.

Documents to be enclosed along with the claim
❖ ORIGINAL Death Certificate. Certificate downloaded from Govt./ Corporation
websites are accepted.

❖ Three (03) passport sized Joint/ Single photographs of all the PENSION claimants –
Widow/ Widower/ eligible children/ dependent parents for unmarried members, duly
attested by the employer mentioning PF account number/ UAN behind the photos.

❖ Date of Birth proof (Education Cert./ Birth Certificate/ Passport) of all children
claiming pension duly attested by employer

❖ Scheme Certificate (if applicable)

❖ Cancelled cheque-leaf bearing name, account number and IFSC or attested copy of
first page of bank Pass Book with all said details clearly visible.

❖ Copy of Aadhar card of all the claimants

Documents to be enclosed along with the claim
❖ Surviving Family Member Certificate (SFC) duly signed by the employer. If parents
are claimants, it should be clearly mentioned whether they were DEPENDENTS of
deceased member or not

❖ Non-Employment certificate (NEC) – to be filled in by claimant w.r.t employment


details prior to the deceased member’s current Date of Joining in EPF Scheme as per the
details available against the estt./ PF account number for which pension is claimed and
also for any missing period of service. NEC should be submitted only for non-employment
period. For any previous employment with PF account, the details of PF Accounts are to be
submitted. NEC should be duly attested by employer also on the basis of Form 11
submitted by deceased member.

➢ Ex1: DoJ : 01.07.2015; DoDeath : 25.07.2020. NEC required for period of non-employment upto
30.06.2015
➢ Ex2 : DoJ : 01.07.2015 (including some previous transfer-in); DoExit : 31.03.2018; DoDeath :
25.07.2020; NEC required for period upto 30.06.2015 and for the period 01.04.2018 to 25.07.2020.
