Introduction
Ward rounds are complex clinicl activities ,critical to providing
high quality ,safe care for patients in timely ,relevant
manner.They provide an opportunity for the multidisciplinary
team to come together to review a patients condition and
develop a coordinated plan of care ,while facilitating full
engagement of the patient and /or caregiver in making shared
decision about care .It offers great opportunities for effective
communication ,information sharing and joint learning through
active participation of all members of multidisciplinary team.
Given the importance of this clinical area we Bele gesgar
primary hospital physician ,nurse ,pharmacist ,lab
technician ,biomedical and env’tal health professional jointly
prepare this ward round protocol .health professionals
understood the wider impact of their approaches to ward
care ,whilst SMT and board bear responsibity to protect time
and resources,enabling all members of multidisciplinary team to
prioritise the ward round.

Objective of This Protocol


To strengthen intra professional relation ship
To strengthen the care provided for patient
To increase the frequency of visiting patient

To strengthen the relation b/n staff and patient
To strengthen scientific based decision making

Nursing Hourly Round Protocol


- All nurse shall round and visit the patient every one
hours and asks any new complaint
- Record if any new complaint
- The nurse has to complete 4P for critical patient on
this round

Multidisciplinary team round


- Nurse shall participate on multidisciplinary team
round on daily bases for stable patient and twice a
day for critical patient
- On grand round the team shall communicate on
nursing care plan, infection prevention, patient care
and pharmaceutical care

Nursing Audit Protocol


- All nurse should prepare themselves to participate in
nursing audit
- Before audit all nurse should have to know their
responsibility

- They has to support audit team in availing necessary
data while nurse audit team conduct audit
- They should implement action plan prepared by nurse
audit team
- They should develops data based QI project if there is
huge gap
Scientific research Protocol
- Nurse should do research regarding with nursing care
and patient care
- They should have to get consent from patient from
- They should have to inform the organization about
their research title and objective of their research
- They should have to present their finding for
institution
- They can ask any necessary support from hospital if
needed

Vital Sign Recording Protocol


Vital sign is the most important indicator of care of
patient which shows the progress of patient . this is why
we prepare this protocol
- Nurse should have to have know the interpretation of
finding they are going to record

- Then they have to introduce themselves to the patient
- They has to prepare vital sign recording material
- They have to inform what they are going to do and
the procedure.
- Then after they get consent from the patient they
have to record vital sign
- The vital sign have to recorded every 4-6 hours for
stable patient
- The nurse should has to record every two hours for
critical patient and as needed
- The measured vital sign has to be recorded on vital
sign sheet
- The nurse has to use local time while recording the
vital sign

Daily Round and Emergency Table Preparedness
Protocol
- Nurses who assigned at Liaison should round and
monitor the bed three times a day
- He/she has to observe and decides available bed/table
- He/she has to communicate with duty and day time
physician on amount of available bed
- He / she has to reserve four to five bed at inpatient
department for Emergency cases
- The nurse who assigned to EOPD should monitor
their bed daily
- They have to communicate with physician and
dispose the patient to appropriate place
