4. Bio-data format is attached herewiih. Altogether three copies of this Bio-data forms are to be filled up conectly and
time of document verification/ interview. One set of all certificates/lestimonials are to be attached with each Bio{ata and Passport

5. The date and venue fixed for interview cannot be changed under any circumstances and no application / representation will be
entertained in this regard except due to Bandh / Picketing etc. in SHILLONG.

6. The Corporation shaii reimburse 3'o Class AC Sleeper Train Fare or Bus fare where train is nol connected from the
mentioned to SHILLONG by the shortest direct route on production of documentary proof/ tickets and an amount of Rs 40001
(Rupees tour tnousanOpnly will be paid as incidental charges. You have to provide the Bank Account details for reimbursement
of travelling expenses in the format as enclosed.
7. please note that if all the documents mentioned in Sl.No. 3 & 4 above are not produced on the date of interview or not fulfilling the
qualification as mentioned in the advertisement / application, you will neither be allowed to appear in the interview nor any Travelling
Fare/ incidental will be reimbursed. Further to it at any point of time any discrepancies/ misrepresentation of facts are detected,
your candidature will be outrightly rejected.
