Dear Mayor Gualberto,

This is in relation to the letter of one of our partners, Parent Advocates for
Visually Impaired Children, Inc. (PAVIC) regarding the Recap Training for our
Barangay Health Workers (BHW) titled “Tap the Gap: A Culminating Activity on
How to Apply Theory and Process in Community-Based Rehabilitation.”
rescheduled on February 8, 2025 (Saturday) with a target participants of 130.

By this, we are respectfully requesting the following:


1. Venue – ABC Hall, Liga ng mga Barangay
2. Overtime pay for one (1) staff of Liga ng mga Barangay
3. Overtime pay for three (3) staffs of Persons with Disability Affairs Office
(PDAO)

To be charged under the PWD Fund.
