Training conducted - week 41/24 AVCS Base DVDs handed over to the Bridge Team. Master must verify all the largest
scale ENCs published for the voyage (by route) are procured/installed/updated & passage planned on them
before approval. Chart agents be contacted to supply the base DVDs every 4-8 weeks. Record the status of the DVDs
in NP133c & verification in ShipSure - NAV 09a.
Training conducted - voyage largest scale ENCs must be manually corrected for the T&P corrections visible on the AIO
layer for the countries listed in the status list & EP corrections for all the areas. Master should define the limits in his
standing order. Reference NP231, 6.1 page 98 & NP232, 11.7 page 59 ‐ enable AIO layer during appraisal & planning,
thereafter, switched off. Master to verify & record the monthly status in ShipSure - NAV 09a.
3. Collision Avoidance:
Only the AIS target was acquired in ECDIS, same target on the radar was not acquired.
Contrary to the Master Standing Order requirements, the target vector is set to True on both radars
Both ECDIS set to speed log (BT) instead of WT even when away from port approaches.
Training conducted - ensure AIS is not used for collision avoidance in both Radar & ECDIS - post label on both ECDIS
too. All close-by targets (TT) on radars must be acquired to assess collision avoidance actions. Only water track (WT)
must be used. Master to verify at random during the voyage & Navigation Self-Assessment (NAV 15).
4. OOW unaware of the visibility range of the Daylight Signalling Lamp during daytime [as per IMO performance
standards Resolution MSC. 95(72)] and/or the range mentioned by the maker during darkness.
Training conducted - range label be posted at conspicuous places so that the OOWs are reminded repeatedly to use
the signalling lamp as required by COLREGs Rule 34 (d).
5. CPA/TCPA:
Alert in both radars not set as per requirement & alert switched off while underway.
Values in AIS set very high (6 NM/60 mins) and the Pop-Up & Buzzer alert switched off.
Values in ECDIS not set as per requirement.
Training conducted - CPA/TCPA values must be set in applicable Bridge Equipment as per Master’s Standing Orders
(MSO) or in the absence of MSO values listed in “VMS/V-NAV-001 Safe Navigation > 20.4 Company Guidance on CPA,
TCPA”. Alert must be enabled. Master to verify at random during the voyage & Navigation Self-Assessment (NAV 15).
Chief Engineer to conduct training of all Navigating Officers, record in Drill & Exercise Report (HSSEQ 52). This drill
must be repeated at regular intervals/change of bridge team.
Training conducted - reference “VMS/V-NAV-004 Navigation Equipment > 7.3.1 Echo Sounder Process & 3.8.1 ECDIS
Maintenance Process”:
Shallow water alarm must be set to present UKC (Position of change to be as per WP List in Passage Plan).
Changes must be marked on ENCs.
Performance checks conducted either by comparison basis calculation (in all ranges) and/or using hand lead
lines in sheltered waters (in shallow ranges) at regular intervals, maintain records.
Master may include requirements of shallow water alarm settings in standing orders, verify at random &
during Navigation Self-Assessment (NAV 15).
8. On the approved passage plan as monitored, XTD values as set not in compliance with the guidance in VMS.
Training conducted - XTD values for the track must be set as recommended by “VMS/V-NAV-004 Navigation
Equipment > 3.6.8 Cross track distance (XTD)”. Master must verify randomly before approval of the passage for
monitoring on ECDIS.
9. Pilotage:
Pilot Card (NAV 03) - no of propellers, time from full ahead to full astern, minimum Steering Speed, Steering
characteristics, helm hard over timing, ship windage area, position of the Automatic Identification System
(AIS) antenna, safe working load (SWL) of bollards either incorrect or not listed.
Master/Pilot Exchange (NAV 04) - Detailed mooring/anchoring plan (including for STS) not listed or attached,
tug details and whether made fast or assisting not recorded in most cases, prevalent/expected weather & sea
conditions not listed, In the Passage Plan section, important considerations during the pilotage such as speed
restrictions, UKC, etc not listed.
Training conducted - Pilot card must be updated with static & filled with dynamic data basis vessel conditions. MPEx
must be in detail to ensure the Bridge Team is fully aware of expected weather conditions & manoeuvring plan.
Master to verify records during arrival/departure & Navigation Self-Assessment (NAV 15).
Training conducted - Improve plan for anchoring & watchkeeping in compliance with “VMS/V-LPG-004 Mooring and
Anchoring Operations”, detailed records must be maintained. Master may amend his standing orders & randomly
verify compliance during Navigation Self-Assessment (NAV 15).
11. No records for the pre-arrival toolbox meeting conducted to discuss safe mooring (including STS)/anchoring
operations before each operation.
Detailed mooring arrangement placard/approved plan by the terminal (or by POAC for STS) must be used by
the officer in charge at each station to conduct a pre-mooring/anchoring briefing of the team. On completion,
required entries be made in the logbook to confirm all the persons involved in mooring/anchoring operations are
briefed on the plan, risks/hazards and control measures to be implemented for safe operations in the prevailing
conditions. Master to verify during Navigation Self-Assessment (NAV 15).
12. Both FMD‐3200/3300 ECDIS are presently installed with software version 3.23, which is not the latest software
version issued by the maker (Furuno) for the model.
As listed/recommended on the maker’s (Furuno) website, contact the local distributor for the latest software version
(05.xx) needed and have it updated, record the upgrade in NP133c.
Training conducted - measure the correct offset values for both GPS, AIS antennas fitted on the Monkey Island. In
consultation with makers, correct the values. Master to confirm settings during Navigation Self-Assessment (NAV 15).
Training conducted - ensure comparison is made by various means, monitored between both gyros on each
observation to identify the accurate one, difference should not be more than 0.5 deg.
15. On the port bridge wing repeater, the rudder angle indicator with 3 deg stbd error.
Investigate & rectify. OOW to check during departure checks (NAV 06) & Master to verify during Navigation Self-
Assessment (NAV 15).
16. Unable to read standard magnetic compass through binnacle at the steering stand.
Investigate & rectify.
17. X-Band Radar “in use” for 7173.5 hrs (magnetron MG5436 replacement recommended by makers Furuno 5000
hrs).
Note: S-Band Radar “in use” for 6635.3 hrs (magnetron MGF5223F replacement recommended by makers Furuno
7000 hrs), PM testing of both radars satisfactory & no defects reported by the Bridge Team.
Reference recommendation by makers, consult with the office for X-Band magnetron replacement.
18. Gyro failure checklist (HSSEQ 20 - D07) is incomplete as it does not list all steps of possible changeover options
and actions needed in all other bridge equipment where gyro input may have failed (only gyro input failure to
ECDIS available).
Training conducted - amend the existing checklist for other heading options (2 nd Gyro, TMC), must also list actions to
amend input in Steering, Radar, ECDIS, AIS, etc. Send to the office for approval, conduct training/drill of the Bridge
Team using the new checklist & record.
19. No records of BNWAS monthly checks in the Deck/Bridge Logbook, and no task is listed in PMS.
Test BNWAS 3-stage alarm, power failure alarm activation by switching off main power supply, backup battery as
required by “VMS/V-NAV-004 Navigation Equipment > 15.1 Bridge Navigation Watch Alarm System (BNWAS)
Process”. Master to verify during the Navigation Self-Assessment (NAV 15). Consult office to add the required task in
PMS.
20. No records determining the accuracy and functional test of the speed log fitted maintained (SIRE 2.0 - 4.1.8).
Training conducted - regularly check for speed log errors during its operation (using maker’s instructions) & record
“VMS/V-NAV-004 Navigation Equipment > 10.1 Speed Log Process & 3.8.1 ECDIS Maintenance Process”. Master to
verify during Navigation Self-Assessment (NAV 15).
Training conducted - all GMDSS equipment (including duplication) must be set correctly to receive warnings. Each
OOW (certified GMDSS officer) must initial the warning messages received during their watches & appropriate
entries in the logbook, plot applicable warnings on the largest scale ENC & inform the Master if dangerous to
navigation.
Training conducted - height correction must be added to the observed value on the Barometer before recording the
MSL pressure in logbook (NP100 section 5.1). Height Correction table (basis drafts) be posted abreast barometer for
height correction values. Recommended annual onsite/remote calibration be carried out by the local PMO (WMO
No. 8 Volume I, 3.4.4.2). Master to randomly verify prior signing daily pages & during Navigation Self-Assessment
(NAV 15).
23. Only ECDIS routes & user charts are backed up in an external hard drive monthly but history, event log, etc are
not.
Training conducted - ECDIS data (routes, history, user charts, etc.) must be backed up every month and recorded in
NP133c “VMS/V-NAV-004 Navigation Equipment > 3.8.1 ECDIS Maintenance Process”. Master to confirm during
monthly ENC verification (ShipSure – NAV 09a), and Navigation Self-Assessment (NAV 15). In addition, ensure
the location ECDIS software installation disk, permits, and AVCS Base DVD is known to all - verified during
the periodical ECDIS failure Drill.
24. VDR:
List of alerts not available near the remote panel (wheelhouse).
OOW is unable to verify if Radar/ECDIS is connected & recording in VDR from the remote panel.
Master to ensure VDR data cannot be tampered with, verify all the sensors/inputs as per VDR compliance report is
connected & recording, alarm list be posted for quick understanding of error alerts. OOW to check during departure
checks (NAV 06) & Master to verify during Navigation Audit (NAV 15).
25. Status of onboard publications maintained in SAF 18 Form, issued in Jul/23 instead of the latest “Onboard
Publication” requirement list issued in Apr/24.
26. Various daily records in the Bridge logbook not entered such as the use of all the checklists in the day, sounding
of ballast tanks/voids/chain locker/etc not recorded, BNWAS on duration, draft, etc
Master & Chief Officer before signing daily pages must verify, and ensure all required entries are made.
27. Old version of Navigation (NAV) forms & defunct Critical Operations Checklists (SAF16) still in use.
Discard all hard copies & electronic unused forms of the old version immediately. Transfer content of checklists in the
VMS ver 1.0.1 company forms and obtain office approval where required. Master to verify during the next VMS
review for compliance.
