STATUTORY DECLARATON

I, MUSANA SAMUEL DAVID of P.O BOX …….. Kampala do hereby make this declaration
and solemnly state as follows:

1. THAT I am a male adult Ugandan of sound mind.

2. THAT I hold a Busia District Birth Certificate bearing my date of birth as 8 August 2000.

3. THAT my academic documents bear my date of birth as 08-August 2000.

4. THAT my driving permit and National Identity Card bear my date of birth as 25 December
1999.

5. THAT I wish to verify and confirm that although the dates on the above-mentioned
documents are dissimilar, those dates refer to no other person other than myself.

6. THAT in so far as the dates “25- December 1999” which appear on my National Identity
Card and Driving Permit AND “08-August 2000.” which appear on my Birth Certificate and
ALL my academic documents are dissimilar, they refer to me and no other person AND I
wish to declare and continue to use 8 August 2000 as my date of birth and shall continue
to use the same at all times hereafter, in all records, documents and other
writings and in all actions and proceedings, as well as in all dealings and
transactions and on all occasions whatsoever.

7. THAT I make this statutory declaration to clarify and confirm that the dates referred to
above refer to me and to no other person despite the dissimilarity.

8. THAT I make this declaration conscientiously knowing and believing the contents hereof to
be true and correct.
