Chapter 01
Understanding Hair Growth
01

Anagen
Also known as the growth phase, this is the longest and most active stage of the
hair growth cycle. During this phase, hair grows at a rate of about 1–2 centimeters
per month.

Catagen
The catagen phase is a brief, 1-2 week transition stage where hair growth stops, and
the follicle detaches from its blood supply, preparing for the shedding phase.

Telogen
This is the shortest and least active stage of the hair growth cycle. Hair shedding
occurs in the telogen phase of the hair cycle. When more hairs than usual enter this
resting phase, it can lead to increased shedding, known as telogen effluvium (TE).

Exogen
This is the final stage of the hair growth cycle, when hair falls out and new hair
replaces it. This phase typically lasts for 6–12 months.

Chapter 02
Does Masturbation Cause hairloss ?

02

No, masturbation does not cause hair loss. This is a myth with no scientific basis. Hair
loss, particularly in conditions like male pattern baldness (androgenetic alopecia), is
influenced by factors such as:
Genetics: Family history is the strongest predictor of hair loss.
Hormones: Increased sensitivity to dihydrotestosterone (DHT), a derivative of
testosterone, contributes to hair thinning and loss.
Age: Hair naturally thins as you grow older.
Health Conditions: Conditions like alopecia areata, scalp infections, or
deficiencies in nutrients like iron or vitamin D.
Why the Myth Exists:
Masturbation temporarily increases testosterone levels, but these changes are
short-lived and have no long-term effect on hair growth or loss.
The myth may have originated from misunderstandings about testosterone,
which is indirectly linked to hair loss in cases of DHT sensitivity.
Key Takeaway:
Masturbation is a normal activity and does not cause hair loss or any other significant
health issues when done in moderation. But this does not mean you close this pdf
and start masturbating

Chapter 03
"Is shedding hair with a Root bulb
normal, or should I be concerned?"
02

The root bulb you’re seeing is a sign of healthy shedding—totally normal! It's actually
a good thing because hair that sheds with a root bulb means it's a natural cycle, not
breakage. If there's no bulb, it could indicate that the hair is snapping off, which isn't
ideal.

Hair shedding is part of the natural hair growth cycle. Each strand eventually "shuts
down" its growth phase, falls out, and makes way for new growth in the following
months. This happens with all hair, as every strand has a terminal length it must reach
before falling out to be replaced.

Daily shedding? No worries—that's completely normal. If you're shedding in larger

amounts over several months, that can still be fine and is known as Telogen
Effluvium. It's usually triggered by stress, illness, hormonal changes, or weight
fluctuations. The good news is that it’s temporary, and your hair will regrow thicker
soon!

However, if you start noticing bald spots, that’s when you might want to look deeper
into things.

Chapter 4
What Causes Hair-fall ?

03

Genetic hair loss, also known as androgenetic alopecia, is a hereditary

condition caused by a sensitivity to dihydrotestosterone (DHT), a byproduct
of testosterone. This sensitivity leads to the gradual shrinking of hair follicles,
resulting in thinner, weaker hair over time. In men, it typically presents as a
receding hairline and thinning at the crown (male pattern baldness). In women,
it causes diffuse thinning across the scalp (female pattern hair loss). The
progression is influenced by genetics, hormonal changes, and age.

Alopecia areata is an Autoimmune disease. It happens

when a person's immune system attacks their own hair
follicles, leading to hair loss. Alopecia areata causes your
hair to fall out in patches.
Treatment : Rx:
Injection: Triamcinolone Acetonide
Concentration: 2.5–10 mg/ml
Inject 0.1 mL per site intradermally.
Total volume: Up to 3 mL (15 mg) per session.
Frequency: Every 4–6 weeks.

Dr. Prithish Bhardwaj

What Causes Hair-fall ?

Hormones have a close

association with hair growth;
thus, they have a big impact on
the hair cycle and hair follicle
structure. Many hormones
control hair growth,cycle, and
density. Harmones like
Androgens, Oestroge,
Cortisol, Thyroid have major
impact on the health & growth
the hair

1) Androgens - Androgens are a group of sex hormones. They

help people enter puberty & mature physically.
Testosterone is the most common androgen. Hair follicles in the
body regions such as face, axilla, pubis and chest grow thicker if
androgens are high but in contrast, androgens will Shrink or Weaken
hair follicles on Scap

Why Does Anil kapoor has more hair in

his Chest and still not Bald ?

Clearly, Anil Kapoor has absolutely loads of

androgen receptors in his chest, but not very
many in his scalp . Stress Leads to activation
of more androgen receptors , therefore
lesser the stress more are the hair

Dr. Prithish Bhardwaj

Let me Build your Concept
No one will Tell you This

Some men have high levels of DHT and still keep a full head of hair.
Take actor Anil Kapoor, for example. Despite having a very hairy
chest (a sign of high DHT levels), he hasn’t lost the hair on his head

Men who are bald or balding tend to have more ANDROGEN

RECEPTORS in their hair follicles. These receptors attract DHT, causing
the hair follicles to shrink.

Understanding this can help us find better ways to address and treat male
baldness. So, while DHT contributes to hair loss, the number of
androgen receptors in the hair follicles is a crucial factor in
determining whether a man will go bald. This process, known as
miniaturization, results in thinner and shorter hairs. Eventually, the follicles
stop producing hair altogether

How does Salman treat Alopecia
Areata ??
Apparently is thought that Salman uses the following
ingredients to treat Alopecia Areata :
Phenol
DPCP (Diphencyprone)
DNCP (Dinitrochlorobenzene)
Sat-be (this seems to be a proprietary or regional
formulation
Croton Oil

Free me dvai lage gi mere

bhai 15 min me dho lena !!
How does it Work ?? Why does he ask to wash in 15 mins ?
The combination of Phenol, DPCP, DNCP, Satbe, and Croton oil for treating alopecia areata :
1. Phenol – While phenol can promote hair regrowth by irritating the scalp, it can also cause
burns, irritation, or scarring if not used properly..
2. DPCP (Diphencyprone) – DPCP is commonly used in contact immunotherapy for
alopecia areata and is generally considered safe when applied by a doctor. It can cause
allergic reactions or skin irritation.
3. DNCP (Dinitrochlorobenzene) – Like DPCP, DNCP is used for contact immunotherapy
and can be effective. It also comes with risks of allergic reactions or irritation, safety
depend on the specific ingredients and concentrations used.
4. Croton Oil – Croton oil is a strong irritant that can cause skin burns or severe irritation if
used improperly. It should be used cautiously and under a doctor's guidance to avoid
damaging the scalp.
5. For Safety it is advised to Remove the chemicals with in 10-15 mins post application to
avoid side effects -
Can it treat Male pattern baldness ?
NO ,While the ingredients you mentioned can be helpful for alopecia areata, they are not
effective for treating male pattern baldness.

Is Salman trying to Scam people ?

I dont think so , he wants to come up with a solution which is unique , his intention are good but
his method and safety standards can be very harmfull

Dr. Prithish Bhardwaj

Oestrogen - High levels of oestrogen can lead to Gynacomastia
(Man Boobs) ,Infertility (reduced sperm production) Erectile
dysfunction, Decreased muscle mass ,Increased fat deposition .
Ab se Hormonal Profile me Oestrogen bhi test karna Hai.

Thyroid -The thyroid hormone plays essential role in the

development & maintenance of hair follicles. Then the body has
too much or not enough thyroid hormone, it can shock the system
‘‘Eelogen Effluvium’’ Hairloss occurs when the thyroid gland is not
working properly. It can be a sign of either hypothyroidism or
hyperthyroidism.

Cortisol - When cortisol levels are elevated due to chronic stress, it

can lead to hairfall. Cortisol can also disrupt the balance or other
hormones in the body, including testosterone & estrogen.

All about Micro-Needling
The Dermastamp is a device that works by

Stimulates Hair Follicles: Creates micro-injuries in

the scalp, activating the wound-healing process and
stimulating dormant hair follicles.
Increases Collagen Production: Promotes collagen
synthesis, improving scalp health and creating a
favorable environment for hair growth.
Enhances Topical Absorption: Improves the
penetration and effectiveness of hair regrowth
serums like minoxidil, peptides, or growth factors.
Promotes Blood Circulation: Increases blood flow to
hair follicles, providing essential nutrients and
oxygen.

When to not Micro-Needle

Contraindications of Dermastamp :
1. Active Infections or Inflammation: Avoid in cases of scalp infections (e.g., fungal, bacterial).
2. Skin Conditions: Conditions like psoriasis, eczema, or severe dandruff on the scalp.
3. Blood Disorders: Avoid in patients with clotting disorders or those on blood-thinning
medications.
4. Pregnancy or Breastfeeding: Safety during these periods is not well established.
5. Recent Surgery or Scalp Injury: Wait until the scalp fully heals.
6. Severe Hair Loss: Not effective in advanced stages of hair loss with complete follicular
destruction.

Myths about micro-needling

1. "Dermastamp Works Overnight": Hair regrowth takes time, typically 3–6 months of consistent
use.
2. "It Can Cause Permanent Scalp Damage": When used correctly, it does not damage the scalp;
3. "Dermastamp Alone Is Enough": Results are enhanced when combined with treatments like
Redensyl Capixyl procapil"It Causes Hair Loss": Temporary shedding (shock loss) may occur, but
it’s part of the regrowth process.
4. "Dermastamp Only Works for Men": It is equally effective for women with certain types of hair
loss.

Chapter 07

Let’s talk Dandruff

04 A-z about Dandruff

Q) WHAT IS DANDRUFF ?

It's a fungus that everybody has on their scalp called Malassezia. It only presents
dandruff symptoms when it grows out of control. Some people are just more
susceptible to dandruff. However, things like stress, age, hormones, and
hygiene can cause the fungus to multiply and dandruff symptoms to get worse.

Q) TYPES OF DANDRUFF ?

Dandruff can be dermatologically categorized into dry dandruff and oily dandruff
(seborrheic dermatitis):

Dry Dandruff: Caused by a dry scalp leading to faster skin cell turnover. It
results in small, white flakes without significant inflammation or irritation.

Oily Dandruff: Caused by overproduction of sebum and the fungus

Malassezia, Larger greasy flakes, intense itching, redness, oily scalp,
Dandruff Will Re-occur after Few Days of Washing it .

Both types involve accelerated skin shedding, but oily dandruff also includes
inflammation from fungal growth, while dry dandruff is typically mild with no
significant irritation.

Q) I FEEL I HAVE PIMPELS IN SCALP . WHAT ARE THEY

ACTUALY ??
Symptoms of Scalp Folliculitis: Red bumps, itching, pus-filled blisters, scalp pain,
crusting, and possible hair loss.

Causes: Bacterial or fungal infections, ingrown hairs, excessive sweating, tight

hairstyles, and poor hygiene.

Let’s do a Test Together

09 Doxab jab bhi balon pe hath marta hun 3-4 to aise hi aajate hain

The hair pull test is a simple clinical diagnostic procedure used to assess active hair
loss. It helps determine the severity and type of hair shedding and can guide further
evaluation or treatment.

How It's Performed:

The examiner grabs a small section of about 40-60 hairs close to the scalp.
Gentle traction is applied to the hairs.
Any hairs that come out easily are counted.

Interpretation:
Normal Result: Losing fewer than 5-10 hairs is typically considered normal.

Abnormal Result:
Losing more than 10 hairs may indicate active hair shedding, often linked to
conditions such as:
Telogen Effluvium (temporary shedding post-stress, illness, or hormonal
changes)
Alopecia Areata (autoimmune hair loss)
Androgenetic Alopecia (pattern hair loss)

Limitations:
It does not pinpoint the underlying cause of hair loss.
False positives can occur if the hair has not been washed or combed recently, as
this can dislodge naturally shed hairs.

Additional Evaluation:
If the hair pull test is abnormal, further investigations like trichoscopy, scalp biopsy,
or blood tests for nutrient deficiencies (e.g., iron, zinc, biotin) may be required to
identify the cause.

Androgenetic Alopecia
14 Male pattern baldness , Female Pattern Baldness , Common
Balding

AGA is the most common type of Non scaring Alopecia , suffered by majority of
the patients.

Mechanism of Action
5-a Reductase

DHT is the metabolite which causes Pattern Baldness . The most Sensitive part of
the Scalp towards DHT is your 1) Crown Area 2) FrontoTemporal Area which shows
miniaturization(decrease in size and area) on exposure to Androgen .

Do all parts of body show miniaturization on exposure to Androgen ?

NO , after puberty effect of Androgen leads to development of body and facial Hair ,
even low Pitched Voice in males and thickness in Adam’s Apple .

How Does the Patient Presents

Males usually presents with :
1) Decline in Hair Growth in Androgen Sensitive Area ( Crown & Hair-line )
2) The terminal end of the Hair becomes hypopigmented , thin , short and
Vellus hair

Females usually present with

1) Thinning of the Crown Area and Widening of the Parting , hair line is
usually intact in females

A minority of men Can also show , preservation of frontal

hair line but there is thinning in the crown(vertex area) .

Men who develop AGA in early age tend to have early and
rapidly progressive Hair loss

Dr. Prithish Bhardwaj

Stages of Hair-Fall

15

How to Confirm Androgenetic Alopecia ?

In a normal male Ratio of Terminal Hair to Vellus hair is 7:1 but in AGA
this Ratio Decreases to 4:1 , to be confirmed on Trichogram.

What to Confuse AGA with ; Differential Diagnosis

Telogen Effluvium : Hair Shedding is Predominant Complaint , rather than
Hair Thinning . Shedding is not limited to just Crown and Hair-line
(Frontotempoal area) . Hair Pull Test is Positive (+) .

Diffuse Alopecia Areata : 8Sudden9 Diffuse Hair Shedding , presenting in a

patchy manner . It may also accompany hairloss in eyebrows and eye lashes
.
Exclamation Hair Sign : Seen on Trichoscopy
Swarm of Bees Sign : Seen on Scalp Biopsy , as lymphocyte cells gather
around the hair Bulb .

16
Treatment for AGA
Treatment of AGA is Highly Subjective , Men who consider baldness to have a
significant impact on their Social , psychological & personal life should be
treated with proper counselling for fair and honest expectation in terms of hair
regrowth and gains

Treatment of AGA is Sub-divided into :

1) Drug Treatment
2) Surgical Treatment
3) Cosmetic Caouflage

Drug Treatment

1) Redensyl Capixil Procapil Aminexil:

RCP Combination is getting as an emerging treatment for AGA . The best part of
peptide based treatment is high acceptance by the patient due to fear of negigible
Side effects . Research have proved , RCP and minoxidil groups revealed that
patients treated with RCP had a significantly higher researcher score (minoxidil
group 25.5% vs. RCP group 64.7%), higher global photographic evaluation score
(minoxidil: 60% vs. RCP: 88.9%), and higher self-evaluation score.

Topical minoxidil (2,5,10%) :

Do you think minoxidil just increases the blood supply to your hair ?
Minoxidil works via the via -
a) Its Vasodilatory effect increases the blood supply to your hair follicle
b)Its angiogenic , mitogenic effect on the germinal cells leads to development of
hair follicle Stem cell .
c) It also has immunosuppresive and procollagenic effect.

Q) Why do We face hair fall on Starting Minoxidil ??

A:This happens because minoxidil speeds up the hair growth cycle, moving hairs
from the resting phase (telogen) to the growth phase (anagen). As old hairs are
shed, it makes way for newer, stronger ones. This shedding phase is usually
temporary, lasting a few weeks to a couple of months, and it’s actually a sign that
the treatment is working.

Oral Finastride :
Finasteride treats androgenetic alopecia (AGA) by inhibiting 5α-reductase type II,
an enzyme that converts testosterone to dihydrotestosterone (DHT), the hormone
responsible for hair follicle miniaturization. By lowering DHT levels by 60-70%,
finasteride slows hair loss and can promote thicker hair growth in AGA. Typically
prescribed at 1 mg daily, its benefits are sustained only with continued use, as
stopping treatment allows DHT levels to return to baseline, often resuming hair
loss progression.

Dr. Prithish Bhardwaj

17
Treatment for AGA
Surgical Treatment

Platelet Rich Plasma : Its in a minimally invasive technique in which Venous Blood is
Drawn from patients body , which is centrifuged to obtain Platelet Rich plasma
which Conatins Multiple growth factor like , vascular Endothelial Growth factor ,
Fibroblast growth factor , platelet derived growth factor which Promote hair
Growth.

Q)How many PRP sessions are needed?

A)Generally, 3-4 initial sessions spaced 4-6 weeks apart are recommended, followed
by maintenance every 3-6 months.

Q)Is PRP therapy painful?

A)Some discomfort is expected with injections, but a numbing agent is often used to
reduce pain.

Q)How long do PRP results last?

A)Results can last up to a year or more, with maintenance sessions advised to sustain
hair growth benefits.

Microneedling: It promotes hair growth by creating micro-injuries that stimulate

growth factors (PDGF, EGF, VEGF), enhance blood flow, and activate the Wnt/β-
catenin pathway—all of which support hair follicle health and growth. It also
improves absorption of topical treatments like Redensyl or other peptides l,
increasing their effectiveness.

Low Level Light Therapy : Some LLLT have been approved by FDA for treatment in
some alopecia’s . it uses red or near-infrared light to stimulate hair follicles, improve
circulation, and promote hair regrowth. It is non-invasive, safe, and effective for
treating androgenetic alopecia. Regular use of devices like laser combs or helmets
can lead to thicker, stronger hair over several months.

Lets Diagnose and Treat a case
of Genetic Hair loss
A 26-year-old male presents with complaints of
progressive hair thinning over the crown and
frontal scalp. He reports oily dandruff associated
with an itchy and flaky scalp, . There are no other
comorbidities or systemic symptoms. The patient
has a positive family history of pattern baldness
(father and paternal uncle). He expresses
significant emotional distress and stress related to
his ongoing hair loss, which he feels is affecting his
confidence and social interactions.

CONDITION ON PRESENTATION
Lets Solve The Case Step by Step
1) Medical History

1st We will take a detailed Medical comprising of :

1) Chief complaint , past History , Drug History , Family History ,
comorbidities & Scalp Health and Mental History

2) Scalp Analysis
Frontal Recession: Visible deep thinning around the temples.

Intermediate Band of Hair: The region separating the crown and

frontal balding areas is still intact but appears thin.

Suggestive of Grade 4 male AGA + SD

Additionally:
The visible dandruff and scalp scaling suggest seborrheic
dermatitis, which can contribute to inflammation and
exacerbate hair loss.

FAQ regarding HT

24
Q)How long does the procedure take?
The procedure typically takes 4 to 8 hours,
depending on the number of grafts being
transplanted and the technique used.

Q)Is the procedure painful?

Most patients experience minimal
discomfort during the procedure, as it is
done under local anesthesia. Afterward,
mild soreness or swelling may occur, but it
is manageable with over-the-counter pain
relievers.

Q)How soon will I see results?

Initial results are visible within 3 to 6 months, with full results appearing in 9 to 12 months as
the transplanted hair grows and thickens.
What are the different types of hair transplant techniques?

FUE (Follicular Unit Extraction)

Hair follicles are taken one by one from the back of your head and transplanted to bald
areas.
Key Point: No big scar, just small dots; heals quickly.

FUT (Follicular Unit Transplantation)

A strip of scalp is removed from the back of your head, and hairs are transplanted.

Simple Difference:
FUE: No big scar, good for small bald spots.
FUT: Leaves a scar, better for big bald spots.
Both methods can give great results based on what you need!

or hair transplant Surgeon

Q)What is the recovery time?

Recovery usually takes 7 to 10 days. Most patients can return to work within a week, but it’s important to avoid
strenuous activities for a few weeks.

Are there any risks or side effects?

While hair transplants are generally safe, there may be risks such as infection, scarring, or unnatural-looking
results. Choosing an experienced surgeon reduces these risks.

Q)How much does a hair transplant cost?

Costs vary depending on the location, surgeon, and technique used, but typically range from
₹50,000 to ₹2,00,000 or more. Cost depends on no of grafts , like 10rs for a graft to 35rs a
graft

Q) Who is a good candidate for a hair transplant?

Good candidates are typically people with stable hair loss, sufficient donor hair, and realistic
expectations. Age, general health, and hair type also play a role in eligibility.

Chapter 15

Telogen Effluvium
Stress Hair-Loss,Temporary Hair Loss , Postpartum Hair Loss

The human scalp has over 100,000 hair follicles, with 90% in the growth (anagen) phase
and 10% in the resting (telogen) phase. Shedding fewer than 100 hairs daily is normal;
exceeding this may indicate telogen effluvium (TE).

TE is a common, non-scarring hair loss caused by triggers like stress, illness( Covid ,
Typhoid, Dengue, Accidents), or medications. It leads to excessive hair shedding and
reduced volume. Acute TE lasts under six months, while chronic TE persists longer .

Clinical Features: Recognizing Telogen Effluvium

Excessive Hair Shedding

Patients with TE often report sudden and heavy hair loss, sometimes collecting shed
hairs (a "Bag Sign") to show the extent. Severe cases, involving over 25% hair loss, result
in noticeable thinning, such as reduced ponytail volume, mostly hair are lost with the
haiir bulb attached Acute TE often follows a trigger (e.g., stress or illness) by 2-3 months,
while chronic TE (CTE) persists longer and may arise from underlying issues like
nutritional deficiencies or comorbidities.

Examination Finding

Hair analysis shows thinner,

weaker hairs with many in
the telogen phase with white
roots

Key Triggers
Common causes include
stress, illness, nutritional deficits, thyroid dysfunction, and medication side effects.

Acute TE
Hair loss begins 2-3 months after a trigger and lasts 4-8 weeks. Regrowth and Hair fall
stops starts within 4-6 months, with full recovery taking up to 12 months.

Chronic TE
CTE lasts over six months and predominantly affects women aged 30-60. The hair loss
is abrupt but does not lead to complete baldness. The course often fluctuates, with
improvement after addressing triggers.

Dr. Prithish Bhardwaj

02

Investigations: Diagnosing TE
Clinical Tests

Hair Pull Test: Positive across the scalp, especially at the vertex. False negatives occur if
hair is recently washed or combed.
Trichogram: Shows >25% telogen hairs, identified by their club-shaped, non-pigmented
roots.

Trichoscopy:
Helps rule out other conditions like androgenetic alopecia, revealing unique signs like
follicular miniaturization.

Blood Tests

Thyroid dysfunction (TSH, T3, T4)

Nutritional deficiencies (iron, zinc, vitamin D)
Hormonal imbalances, especially in women
Other Tests

Scalp biopsy (rarely needed)

Blood counts and organ function tests to identify systemic causes
Treatment: Managing Telogen Effluvium
Address the Cause
Identify and eliminate triggers such as stress, illness, or nutritional deficiencies.

Iron supplements for ferritin <70 mg/ml

Thyroid hormone therapy if dysfunction is present
Medications

Minoxidil: A 2%-5% topical solution applied twice daily to stimulate regrowth, especially in
chronic or drug-induced TE.
Supportive Care

Balanced nutrition and stress management aid recovery.

Monitor progress regularly for adjustments in treatment.

Differential Diagnoses

Androgenetic Alopecia: Gradual thinning, unlike the abrupt shedding in TE.

Diffuse Alopecia Areata: Autoimmune cause with patchy hair loss.

Anagen Effluvium: Sudden loss during the growth phase, often from chemotherapy.

By identifying triggers and providing timely interventions, TE is usually reversible,

restoring hair volume and confidence.

Blood Work For Hair-fall

Doxab , which Test should I take for Hair Fall

1. Complete Blood Count (CBC):

Hemoglobin: <12 g/dL (women), <13 g/dL (men) suggests anemia.
MCV: <80 fL (iron deficiency), >100 fL (B12/folate deficiency).

1a) Liver Function Tests (LFT) for Hair Fall

ALT/AST: >40 IU/L suggests liver dysfunction, potentially affecting hair
metabolism.
Albumin: <3.5 g/dL may indicate nutritional deficiencies linked to hair loss.
Bilirubin (Total): >1.2 mg/dL could signal underlying systemic issues.

2. Thyroid Function Tests:

TSH: >4.5 μIU/mL (hypothyroidism), <0.5 μIU/mL (hyperthyroidism).
Free T4: <0.9 ng/dL (hypothyroidism), >2.5 ng/dL (hyperthyroidism).

3. Iron Studies:
Ferritin: <30 ng/mL indicates iron deficiency.
TIBC: >450 μg/dL suggests low iron stores.

4. Vitamin Levels:
Vitamin D: <20 ng/mL (deficient).
Vitamin B12: <200 pg/mL (deficient).
Zinc: <70 μg/dL affects hair health.

5. Hormonal Tests:
DHEA-S: Elevated (>430 μg/dL in women) suggests PCOS.
Testosterone (Total): >70 ng/dL in women indicates androgen excess.
Cortisol: >23 μg/dL indicates stress-induced hair loss.

6. Liver & Kidney Function:

Liver Enzymes (AST/ALT): >40 IU/L suggests dysfunction.
Creatinine: >1.2 mg/dL indicates kidney issues.

7. Autoimmune Markers:
ANA (Titer): >1:40 indicates autoimmune causes like alopecia areata.

Chapter 17
28 My secret Power of
Diagnosis in CBC Report
Diagnose Deficiencies in few Seconds

Step 1: What is the Mentzer Index?

The Mentzer Index is a calculation used to differentiate iron deficiency anemia from
thalassemia trait. It is calculated using the formula:

Mentzer Index = RBC Count (Red Blood Cell Count)

= +-13
MCV (Mean Corpuscular Volume)

MCV: Indicates the average size of red blood cells. Normal is about 80–100 fL.
RBC Count: Measures the number of red blood cells in millions per microliter.

Step 2: How to Interpret the Mentzer Index

1. Mentzer Index < 13:
Suggests thalassemia
Patients with thalassemia often have a normal or high RBC count despite having
small red blood cells (low MCV).
2. Mentzer Index > 13:
Suggests iron deficiency anemia.
In iron deficiency, both MCV and RBC count are reduced due to a lack of iron
needed for red blood cell production.

Step 3: What About Vitamin B Deficiency?

Vitamin B deficiency impacts MCV differently:
MCV > 100 fL (macrocytic anemia):
Suggests a deficiency in vitamin B12 or folate.

Practical Case Examples:

Case 1:
MCV: 68 fL, RBC: 5.5 million/µL Mentzer Index = 12.4 Thalassemia Trait.( RBC count will be High)
Case 2:
MCV: 72 fL, RBC: 3.5 million/µL Mentzer Index = 20.5 Iron Deficiency Anemia.( RCB will be low )
Case 3:
MCV: 110 fL, RBC: 2.8 million/µL Macrocytic Anemia Check B12/folate.( RBC will be Low )

HbA1c <7.5% (Mild hyperglycemia): it is Highly reversible .

First-line therapy: Lifestyle modifications (diet and exercise) + Metformin.

Metformin is the preferred initial drug of choice due to its efficacy, safety, and minimal risk of
hypoglycemia ( Drop in Blood Glucose Level below Normal on Consuming)
Follow-up: Assess response in 3 months. If HbA1c is still above target, consider dual therapy

HbA1c 7.5%-8.9% (Moderate hyperglycemia):

Dual therapy: Metformin + second agent based on patient profile:
Sulfonylurea (e.g., glimepiride): If cost is a concern and no risk of hypoglycemia.
SGLT-2 inhibitors (e.g., empagliflozin): For cardiovascular or renal benefits.
DPP-4 inhibitors (e.g., sitagliptin): For weight neutrality and low hypoglycemia risk.
GLP-1 receptor agonists (Semaglutide): For added weight loss benefits.
GLP-1 RA , slows food passage through your Stomach , makes your brain think you
are full & you tend to consume less food = weight loss + control of Sugar as well
(layman Expl.)
Follow-up: Reassess HbA1c after 3 months. Adjust therapy if needed.

HbA1c 9% (Severe hyperglycemia):

Symptomatic or ketosis: Initiate insulin with or without OHAs.
Non-symptomatic:
Triple therapy with Metformin + two additional agents:
SGLT-2 inhibitor + DPP-4 inhibitor or
SGLT-2 inhibitor + sulfonylurea.

For weight loss, GLP-1 receptor agonists can replace one OHA.
Monitoring and Adjustments:
Monitor HbA1c every 3-6 months.

Adjust therapy based on HbA1c targets, side effects, and patient preferences.

Special Considerations:
Elderly or frail: Use agents with low hypoglycemia risk (DPP-4 inhibitors or SGLT-2 inhibitors).

Obesity: Prefer GLP-1 receptor agonists or SGLT-2 inhibitors for weight management.

Cardiovascular disease: Prioritize SGLT-2 inhibitors or GLP-1 receptor agonists.
