1 Defects / Non-Routine Jobs

A Defect may be identified during Operations, Checks, Internal and External audits/inspections,
incidents, etc. These defects range from simple minor defects to those which are significant and need to
be added to Dry Dock List.

1.1 Key action items / Metrics

Following are the measures in Defects/Non-Routine Jobs panel:

FIELDS DESCRIPTION/FUNCTIONS
Draft Displays the list of jobs in draft status
Risk Assessment Pending Displays the list of jobs pending RA
Defects Pending Closure Displays the list of defects pending closure
Dispensation Pending Displays the list of defects that are in dispensation pending
Defects Overdue Displays the list of defects that are overdue in 30 days
Machinery Incident Pending Displays the list of defects pending machinery incident
New Comments (V) Displays the list of vessel’s new comments
New Comments (O) Displays the list of office’s new comments
1.1.1 Defect List
The Defect List menu displays the defects and unscheduled jobs of the components.

User can view the defects and unscheduled jobs of components by clicking on the Defects/ Non -
Routine Jobs button in PMS panel.

1.1.2 Icons in Action Column


Icons and its functions.

ICONS DESCRIPTION
Add: Allows the user to add a new defect.
Edit: Allows the user to edit the defect.
Note: Editing can only be done when the defect is in Draft status.
Delete: Deletes the existing defect.
Create RA: Allows the user to create an RA and prompts a pop-up window to either map
an existing RA or create a new one.
Note: Icons will automatically change colour based on each status.
Dispensation Pending: Displays that the defect’s dispensation is pending and prompts a
pop-up window, allowing the user to update the dispensation dates.
Note: Icons will automatically change colour based on each status.
Machinery Incident to be created: It will prompt a pop-up window and allow the user to
record whether the machinery is damaged.
Note: Icons will automatically change colour based on each status.
Attachment: Allows user to add attachments to the defect.
Comments: It will prompt a pop-up window and allow the user to comment.
Shopping Cart: Displays the list of requisitions created under that particular defect.

Complete Simple Defect: Displays the list of simple defects.


Job History: Displays the list of closed job history.
1.2 Recording a new Defect
A new defect can be recorded from the following sources:

a. While reporting a PMS Work Order.


b. While carrying out RCA / CAR during Incident investigation.
c. While carrying out RCA / CAR for an NC / Observation.
d. While submitting a PMS Report where one or more parameters are out of limits.
e. Directly in the Defect module as shown in the steps below.

1.2.1 Adding New Defect/unscheduled Job


User can add a new defect/unscheduled job for a component with the below steps:

1. Click on the Add icon in Defects/Unscheduled Jobs screen.

2. Select ‘Defect/Non-Routine Job’ for Type.


3. Enter the Title of Defect and Defect Details.
4. Select the Nature of Defect from the drop-down list menu.
5. Select the Report Date and Due Date from the calendar pop-up.
6. Select the Responsibility and Maintenance Class from the drop-down list.
7. Select ‘Select from Existing Job/Create New Job’ for ‘Do you want create new job?’
8. Select ‘Yes/No’ for Dry Dock Job.
9. Select ‘Yes/No’ for ‘Is the defect on Navigation, Safety, or Pollution prevention equipment?’
10. Select ‘Yes/No’ for ‘Significant defect - RA Required?’. RA Create hyperlink will be enabled.
11. Select ‘Yes/No’ for ‘Is Flag / Class to be notified to obtain dispensation?’
12. Select ‘Yes/No’ for ‘Do you want to raise Machinery Incident?’
13. Click Save.

1.2.2 Data Entry – Key points


a. If Do you want PMS job? is selected as 'Yes', the option Do you want to create a new job will be
enabled, providing radio buttons for selecting from existing jobs or creating a new one.

b. If Select from Existing Job is selected, Select Job icon and Clear icon will be enabled in
Action Required.
c. Clicking on the Select Job icon prompts a pop-up window with the existing defect jobs. User
can select the required jobs from the list.

d. After selecting the required job from the list. It will be bound and displayed in the Action
Required field in the Add screen.
e. Is the defect on Navigation, Safety, or Pollution prevention equipment? is automatically filled
by the application based on the Component.
f. If Significant defect - RA Required? is selected ‘Yes’. RA Create hyperlink will be enabled.

g. The RA Create hyperlink prompts a pop-up window with existing non-routine RA lists. Users can
select from these existing lists or create a new one by clicking on the Add icon.
1.2.3 Machinery Incident

The Machinery Incident icon prompts a pop-up window and allows the user to record whether the
machinery is damaged. User can add the incident and click Raise Incident.

1.2.4 Comments
User can add a comment to the defect/unscheduled job with the below steps:

1. Click on the Comments icon under the Action column adjacent to the required component.

2. Enter the comments in the Post Your Comments Here field.


3. Click Choose File to upload attachments (if any).
4. Click Save.

The comment will be added to the list.


1.2.5 Creating a Requisition for a Defect / Unscheduled Job
By clicking on the Create Requisition hyperlink prompts a pop-up window for creating a requisition.

1.3 Closing out a Defect


Closing out a defect involves completing the necessary tasks to resolve the issue identified during
routine maintenance checks. Once the defect has been addressed and verified, it can be closed out as
follows:
a. Defect with PMS Job and Work Order
b. Simple Defect (No PMS jobs)

1.3.1 Defects with PMS Job and Work Order


Defects for which a PMS Job and Work order is created is closed by completing the Job and Work order.
The date of completion of the Work Order and Job is the date of closure of the defect.

1.3.2 Simple Defects (No PMS jobs)


Defects for which a PMS Job and work order is not created are classified as “Simple Jobs”. In the Defect
list an Icon (Complete Simple defect) is provided.
Click on the Icon, select the Done Date, enter action taken in the “Comments” box and then click the
“Save” button. The defect will get closed.
