ACTIVITY COMPLETION REPORT

Name of Activity: PROJECT GRADE (General Report and Analysis of Data on Exams)
 IDENTIFYING INFORMATION
Proponent Unit: Canduhao Elementary School
Date: JULY-DECEMBER 2024 (QUARTER 1 – QUARTER 2)
Venue: Canduhao Elementary School
Participants’ Description:  Teachers, learners and school head.
 ACTIVITY:
Description: The project involves the development and implementation of an electronic
tool used by teachers across all grade levels and subject areas to evaluate
learners' performance in quarterly exams. This tool tracks the number of
mistakes made by each learner, computes their scores, proficiency levels, and
calculates the Mean Percentage Score (MPS) for each subject area. It also
helps teachers identify areas where learners struggle by analyzing the items
with the highest number of mistakes. The tool simplifies data consolidation,
allowing school heads to efficiently generate reports on exam results and make
informed decisions.
Objectives: 1. To utilize an electronic tool for evaluating and tracking learners'
performance in quarterly exams.
2. To compute individual scores, proficiency levels, and Mean
Percentage Scores (MPS) for each subject area.
3. To identify areas of weakness by analyzing common mistakes and
least learned skills.
4. To simplify data consolidation and report generation for school heads.
5. To improve the efficiency of assessment and decision-making in
educational settings.
Preliminaries: The project started with the development of an electronic tool at the beginning
of the school year to help teachers evaluate learners' performance in quarterly
exams. It is then consolidated by the school heads to analyze and generate
reports. This is done quarterly for the whole year.
Design of Activities: 1. Introduction to the tool, its objectives, and functionalities.
2. Teachers learn how to input data, track performance, and generate
reports.
3. Teachers input exam data, and the tool calculates scores and
proficiency levels.
4. School heads generate reports and analyze results.
5. Gathering feedback from teachers and school heads for tool
improvement.
Methodology:  Development Phase: The tool was developed using appropriate
software and technologies.
 Training: Teachers were trained on how to use the tool effectively for
inputting data, analyzing results, and generating reports.
 Implementation: The tool was implemented in classrooms, and
teachers used it for quarterly assessments.
 Data Collection: The tool tracked performance, including mistakes,
proficiency levels, and MPS.
 Analysis: Reports were generated by school heads to assess learner
progress and areas needing improvement.
 Feedback and Adjustment: Continuous feedback from users was
collected to refine the tool.
Management of the School head regularly check-ins with teachers ensure proper tool usage and
Activity: gather feedback. Ongoing assessment of the tool’s effectiveness in tracking
and improving learner outcomes.
Resource Persons: The school head is the tool developer, training facilitator, and subject matter
expert.
Logical Arrangement: Utilization of the tool is required to be submitted as a mandatory report after
quarterly exams.
 RESULTS
Participants’ Feedback:  Teachers reported that the tool made it easier to track learners' progress
and pinpoint areas that needed attention.
 School heads appreciated the tool’s ability to generate detailed reports,
making decision-making more efficient.
 Some teachers suggested additional features for future updates, such as
more detailed analysis options.
Audience participation: Teachers actively participated in training sessions, providing valuable
feedback.
School heads engaged in the report generation process, discussing results and
making decisions based on the data.
Outputs:  Comprehensive reports on learners' performance, including scores,
proficiency levels, and MPS for each subject.
 Collected feedback from teachers and school heads to improve the
tool.
 Actionable insights to address areas of weakness in learners’
performance.
Issues and Concerns:  Ensuring that teachers input data accurately was crucial for generating
reliable results.
 Some teachers faced challenges with the tool’s interface during the
initial stages.
 Continuous training and support were needed to ensure effective use of
the tool.
Recommendations:  Enhance the tool’s user interface to make it more intuitive.
 Consider adding more detailed analysis options, such as breakdowns
and trend analysis for male and female.
 Continue gathering feedback from teachers and school heads to make
further improvements. Enhance audience engagement through
additional interactive activities and better promotion of the event.
