SAS2000 COMMITMENT FORM AY 2024 2025-2-output
1. Student Affairs Services (SAS2000) is a non- academic, zero- unit student formation offered
to all students. To complete your academic requirements, you must get a PASS grade in
SAS2000. Failed grades will forfeit your chances to be on the dean's list (Student
Handbook 11.1.3) or be awarded Latin honors during graduation (Student Handbook
12.4.4.)
2. To pass SAS2000, students must attend three (3) sessions with their SAS2000 resource
persons (F2F and online) and submit the following requirements on or before their due date.
a. Fully accomplished and signed SAS2000 Commitment Form (this document)
b. SAS1000 Reflection Paper
c. Profile Career Keys (PCK) Student Registration
d. 3 SAS2000 learning module certificates
e. Profile Career Keys (PCK) Score Results
Instructions and the due dates of these requirements are posted in your AnimoSpace.
Please ensure you turn on the notification of your AnimoSpace's Announcement section as
all instructions will be posted there.
3. Any concerns on the subject will be coursed through your SAS2000 resource persons.
Please make sure you know their name and email addresses.
4. All concerns related to the modules must be directed through the module developers. The
module developers are the offices that will be offering SAS2000 modules. Please note the
names of the module developers and their contact details. Details such as Zoom link,
registration, evaluation form, andother information must also be directed to the module
developers.
5. You are also NOT allowed to give or share any information about them with your
classmates andfriends. Important information such as Zoom links, evaluation links, google
forms, links, and other information and materials are sent to the individual only.
Furthermore, the falsification of module certificates is strictly prohibited. If proven, the
unauthorized sending of module information and falsification of certificates are grounds
for sanction and filing of a case in the Student Discipline and Formation Office (SDFO).
6. SAS2000 students are expected to communicate with their resource persons and module
developers regarding any inquiries. It is their responsibility to complete the requirements,
schedule their modules, and communicate to their resource persons about concerns about
their course assignments and to the module developers for all module concerns.
As a SAS2000 student, I have read and fully understand my responsibilities in the course,
including its requirements and timelines, and I agree with them.
