Completing the Grade 9 School Immunization Consent Form

Students and their parents / guardians / representatives are encouraged to discuss consent for immunization
together.

Review:
☐ Grade 9 Immunizations in BC HealthLinkBC file
☐ The Infants Act, Mature Minor Consent and Immunization HealthLinkBC file
☐ Grade 9 Catch-up Immunizations resource
☐ Other immunization information (if included)

On the ‘Consent for School Immunization’ form:


☐ Check the name, address and any other personal information to make sure it is correct, and add any
missing information.
☐ Read and complete the Parent / Guardian / Representative Section on the front of the form.
☐ Review the Immunization History on the back of the consent form. If the information in this section is
not up to date and your child has received any vaccines outside of Interior Health Public Health, please
provide a copy of the student’s immunization record to your local health centre. A copy can be attached
to the consent. Visit https://immunizebc.ca/immunization-basics/immunization-records for tips to locate
immunization records.

Consent for immunization is valid for 2 years, unless the parent / guardian / representative or the student cancels it.
Refusals are reviewed regularly. Vaccines may be re-offered at another time or as part of a public health response.
Students will get a record after their immunizations. Students can access their immunization records through Health
Gateway: https://www2.gov.bc.ca/gov/content/health/managing-your-health/health-gateway

Mature Minor Consent


Parents, guardians or representatives should be aware that students can make an informed decision regarding
immunization regardless of parental consent or refusal. This is referred to as “mature minor consent.” Grade 9
students will be informed about immunization and given the opportunity to consent to or refuse immunization.
Please refer to “The Infants Act, Mature Minor Consent and Immunization” health file for more information.
