❖ Introduction :
➢ Definition:

➢ Drug distribution in a hospital means " Physical transfer of drugs from storage area in the
hospital to the patient's bedside"

➢ The procedure of Drug distribution may be categorized into 2 groups:


❖ Dispensing of drugs to Inpatient :
➢ In-patient:

➢ Inpatients are those who get hospitalized for the purpose of treatment of Disease, surgery
and rehabitation.
➢ The pharmacist who involved in dispensing of drugs in out patients can also dispense drugs
for in patients.
➢ The pharmacist who involve in drug distribution of in patient should be well skilled & Qualified.

➢ Drug Distribution Inpatients have 4 categories :

1. Individual prescription order system

2. Complete floor Stock System

3. Combination of Above both Systems

4. Unit Dose Dispensing Method

Drugs Distribution in Hospital


❖ Individual Prescription Order System :
➢ It is a prescription system where physician writes the prescription for individual patient who
obtains the prescribed drug from any medical store or hospital dispensary by paying
own charges.

➢ This System is mainly used in small & private hospitals.

➢ Advantage : I) all medication are directly reviewed by


Pharmacist.
Ii) Medications error avoid.

➢ Disadvantage : I) Increase the cost of patients.


Ii) Not use in big Hospital

Prescription Order System


❖ Complete Floor Stock System :
➢ In this system, both pharmacist & nurse are responsible for drug distribution to patient.
➢ According to this system, Drugs are stored at the nursing station and administered by nurse
according to order of physician.
➢ Drugs are stored on the floor or wards.

➢ Drug dispensed in this system in 2 Categories :

➢ Charge Floor Stock Drug : In this system patient is charged for


Floor Stock Drugs
Every single dose administered to him.

Eg: Antihypertensive drugs, antibiotics, anticoagulant etc.


❖ Complete Floor Stock System :
➢ Non-charge Floor Stock Drug :

➢ Drugs are dispensed to all patients on floor on non-charged basis. Not direct
charges required from patients .

➢ In this system drugs distribute in 2 methods :

➢ Drug Basket Method: Drug delivered through buskets


to floors.

➢ Mobile Dispensary Unit: It consist of specially constructed


stainless-steel trolley
( 60 inch high, 48 inch wide )

➢ Advantage : i) Drugs are readily available for administration


ii) Minimum return of Drugs.
Iii) Reduce in-patient prescription order.

➢ Disadvantage : I) Increase chance of medication error.
Ii) Increase workload.
❖ Combination & Unit Dose Dispensing System :

➢ Combination of Individual prescription order & floor stock system are followed in the govt and
Private hospitals who run on the basis of no profit and no loss.

➢ Unit Dose Dispensing :

➢ Unit dose medication are those which are ordered, packaged & administered in single or
Multiple unit containing predetermined amount of drug and doses.

Eg: One tablet or capsule.

➢ Advantage : I) Better financial control.


Ii) Prevent loss of partially used medication.

➢ Disadvantage : I) Regular monitoring required.

❖ Dispensing of drugs to Outpatient :
➢ Out-patient: ​Out-patient refers to the patients who are not occupying beds in hospital or
In clinics, health centers and other place.

➢ No any medicament should issued without the prescription.

➢ The quantity of drugs supplied should be recorded.

➢ It is also known as Ambulatory Service.

Ambulatory Service
➢ Categories: The categories are -

1. Emergency Outpatients

2 . Referred Outpatients

3. Special Outpatient

4. General Outpatients

❖ Dispensing of Controlled Drugs :

➢ Controlled substance is a drug or chemical which ownership, manufacture or use is


absolutely regulated by a respective government of country such as illegally used drugs
or prescription medication.

➢ ​This laws inforcement leads to prevention in the unauthorized used of drugs such as
central analgesic (Opioids), Anaesthetics, Steroids etc.

➢ List:

➢ Schedule 1: High potential for abuse & have not accepted medical use in treatment
Eg: raw opium

➢ Schedule 2: High potential for abuse but accepted for medical use in treatment
Eg: Cocaine, Morphine

➢ Schedule 3-5: less potential for abuse & accepted for medical use in treatment
Eg: Benzodiazepine & pain killers

➢ From this topic we can conclude that,

➢ The drug distribution system in hospital is Important in providing high-quality care of


patients.

➢ It plays crucial role in ensuring the safe & effective delivery of medicines to the patient.

➢ Errors occur in ward pharmacy system can be reduce if medication is stored in individual
Patient medicine cabinets.
