I declare under oath that I have personally accomplished this REGISTRATION FORM which is a true, correct and complete
statement pursuant to the provisions of pertinent laws, rules and regulations of the Republic of the Philippines and of the City
Ordinance 2015-22. I authorize the agency head/authorized representative to verify/validate the contents stated herein. I agree
that any misrepresentation made in this document and its attachments shall cause the filing of civil/criminal case/s against me.

INSTRUCTIONS
1. SECURE THIS PET REGISTRATION FORM AT THE CITY AGRICULTURE OFFICE OR THE
VETERINARY UNIT.
2. ACCOMPLISHED THE REGISTRATION FORM WITH REQUIREMENTS ALREADY ATTACHED

REQUIREMENTS

FOR OWNER
- Duly accomplished Application Form;
- Certificate of Residency from the barangay;
- 2 pcs. 2x2 photos
- Registration Fee of Seventy Five Pesos (Php 75.00) per pet.

FOR PET
- Certificate/Proof of Anti Rabies Vaccination for the last six (6) months;
- In the absence of a proof of anti - rabies vaccination, the physical presence of the
animal shall be required for proper administration of the required vaccine;
- Full – Body AND Side - View photograph (in 3R) of the animal. (2 PIECES EACH)

UPON RENEWAL (YEARLY)


- Updated Information Sheet
- Renewal Fee of Seventy Five Pesos (Php 75.00)
- Proofs of vaccination for the previous year.
For failure to present such proofs, a fine of One Hundred Pesos (Php 100.00) shall be
charged in addition to the renewal fee.
- In the absence of a proof of anti - rabies vaccination, the physical presence of
the animal shall be required for proper administration of the required vaccine.

3. A certificate of registration shall be issued with the pet tag which must be
displayed on the registered pet at all times.
VIOLATIONS:
a) Failure to register or renew registration of cats and dogs in accordance with the Code and
IRR;
b) Keeping of dogs that have been given anti - rabies vaccines or to keep dogs the last anti
- rabies vaccination of which is already more than one year old;
c) Allowing animals to Stray or roam in public places, streets or plaza;
d) Taking pets out for a walk or on strolls without proper muzzling or restraints;
e) Opposing, resisting or otherwise interfering with an Animal Control Officer or any of his/her
authorized deputies;
f) Failure of pet owners to report animal bite cases; and
g) Failure to clean up animals’ feces and urine in public places.

PENALTIES:
First Offense - A fine of Five Hundred Pesos (Php 500.00) and confiscation of materials used in
the commission of violation.
Second Offense - A fine of One Thousand Pesos (Php 1,000.00) and confiscation of materials
used in the commission of violation.
Third Offense - A fine of Two Thousand Pesos (Php 2,000.00) and confiscation of materials
used in the commission of violation and/or of imprisonment of six (6) months or both, subject to
the discretion of the Judicial Court.
