Manual Excavation by hand.  Potential damage to  Work permit issuer shall ensure that underground

have been located and marked before excavations
start.
 Work scope (purpose, size, and location of
excavation) shall be discussed thoroughly with the
crew.
 Follow the recommendations of TSU in case of any

and for making trial pits. PPEs such as safety

identified, SA issuer and the Technical Support
Unit shall be informed immediately.

kept away/control 1.5 m from excavated area.
 Provide cover/checker/steel plates over the
excavated surface, for easy flow of traffic and
personnel movement.
 Any accident shall be informed to SA regardless
how minor it is.
 In case of any emergency, stop all activities and
proceed to the pre-defined assembly point in that
area.
 Hard Barricade should be provided around
excavated area.
 Provide shoring if required in loose soil area.
 Make proper arrangements to prevent water
logging because of rain.

administrative (procedures) and then PPE.
4. Identify any recommended actions required, e.g., develop a new procedure or perform an
industrial hygiene assessment. Assign a responsible party for any required actions.
5. List each type of PPE needed for the task. Be specific.
6. Identify key steps as “STOP WORK TRIGGERS” (in the “Controls” column) if there are any
conditions, job changes or distractions that will prompt personnel to stop all work activities
associated with the job until further assessment is conducted or additional controls.
7. T&I Critical Activities check list (Supplement #3) SMS#6.06 needs to be filled, verified and
endorsed by operation T&I representative daily with work permit.
