1.0 Introduction (Describe purpose of the work, Give details of type and scope of work being carried
out; work parties involved (include working hours).Client requirements/specifications, Local legislation
Enforcement authorities requirements i.e. The Police, Civil Defence, Environment Agency* (EIA) to be
considered with reference to scope of work.
M/s. PRIYA ENTERPRISES understands the opportunity is being awarded, with this we also understand the
type of work we will be carrying and details about it.
Work will be carried by the professionals and we will ensure the deputation of best experienced taskforce.
The Parties involving will be M/s. PRIYA ENTERPRISES. The working hours will be as per validator.
Client’s requirements are the first priority given on and at site.
We will make sure the on- going work will be as per validator / guidance.
Local authorities and TATA POWER TROMBAY permission will be taken in advance / informed.
PTW and TATA POWER TROMBAY to be considered with reference to scope of work.
2.0 Location of Work (Give site address and precise location on site where work is to be carried out. Give
details of any other locations in the proximity of the works which may affect the safety of the work or
be affected by the work considering Surrounding Environmentand what measures will be used to
control these risks e.g. segregation, warning notices, fencing,Structural stability precautions)
Site Location is TATA POWER TROMBAY, Mahul Vill, Chembur, Mumbai – 400074.
3.0 Safety Document /Specific Approval Required (Details of any safety documents or specific
approval (i.e. Client specific approval) required to undertake the work)
4.0 Access & Egress to the work area: Give details of any special access or egress arrangements (i.e.
Ladders/MEWPS/Scaffold/Trestles/Step Ladder etc.) that will be used including any
obstruction/restriction in safe access to work area. Consider the site access from the public road onto
and around the site before vehicles arrive on site, taking into account the site traffic management
plan, the designated work area for delivery vehicles and the storage area for materials.
1. Designated area is provided with safe passage for access & egress Area to barricade to rest of the
movement of other agencies and barricaded area to be declared as HARD HAT AREA.
2. Route for shifting of materials to be identified and inspected by Site in Charge, Safety Officer &
Site Engineer.
3. Proper back filing of the area to be ensured.
SCAFFOLDING Validator/Engineer:
1. Check the weather conditions before starting the job/work during the work.
2. Check fitness of workers visually. The fitness certificate of each labor shall be collected from us
and verified by the site manager.
3. Access and egress to the work point will be finalize & communicated to all the workmen during
TBT.
4. Check the material availability with tested tools & tackles with proper lifting machinery.
5. Continuous monitoring of activity and random supervision barricading of work area before
start of the job and work area.
6. Proper housekeeping in work area.
Vendor Company/Contractor/Supervisor/Engineer:
1. Participation in tool box talk lead by M/s. PRIYA ENTERPRISES.
2. Work permit to be received from TATA POWER TROMBAY to M/s. PRIYA ENTERPRISES.
3. Cross check the location before start of the job/work.
4. Continuous supervision throughout the job/project till TATA POWER TROMBAY.
5. Ensure that no unsafe act is performed at any time.
6. Follow all instructions on OHS as briefed by TATA POWER TROMBAY from time to time.
7. Ensure the method Statements & Risk Assessments strictly followed.
Safety Officer:
1. Verify OHS arrangements agreed between permit issuer & permit receiver.
2. Check the weather conditions before start the work.
3. Arrange tool box talk with staff & workers before starting the job/work activity for the day.
4. Check fitness of workforce visually.
Operative/Workers:
1. Participate in tool box talks.
2. Follow all instruments & safety guidelines given by supervisors.
3. Ensure the cleanliness and hygiene of work place/ access.
4. Use of correct personal protective equipment as directed and report any deficiencies to their
supervisors.
5. Report all incidents/ near misses to supervisor.
To take care of colleagues and raise alarm whenever any unsafe act is noticed.
6.0 Working/Activity Description: -It is important that all operatives should have clear idea of those
operational sequences and responsible supervisor must verify their competency prior to their engagement
in operation.
6.1Pre-Working Checks

manpower allocated to the task, e.g. titles, qualifications, competences, direct manpower,
contractors. Details of plant, tools and equipment to be used for the work, including the availability of
relevant statutory documents, checks or inspections etc. Details of fencing, barriers, cones, chains,
dangers notices, warning signs etc.
6.3 Details of Permits to Work required for activity: List out applicable Permits in sequence of work,
for e.g. PTW (Permit to work), HIRA, JSA.

checking the safety requirements and verification of the location.

6.4 Operational Sequence of work (i.e. Test Procedure):- Full description of the work, setting out the
methodology in a sequential manner, including any reference to any identified operational restraints.

6.5 Isolation Requirements: -Before starting the work, identify the work area demarcation and
accordingly isolate it from other part of operational interference. Responsible supervisor should cross
verify adequate isolation arrangements for safe operation prior to receive Permit to work and if PTW
not applicable then before start of operation during pre-check survey. Accordingly adequate caution
tag/signs to be provided.
6.6 Work Boundaries/ Schedule of authority clear description of the work boundaries, e.g.
delineation details or terminal points etc. supported where appropriate with sketches or drawings.
Access and egress should only be gained by designated access/egress points. Under no circumstances,
any operational personnel will not be allowed to work beyond their defined schedule of authority as
defined in Permit to work or their defined operational roles/responsibilities. Otherwise such breach
should be considered as procedural violation.
NO Deviation is allowed with respect to approved Risk Assessment & Method Statement. In case
of any difficulty for the practical reasons at any location, any deviation shall be discussed with site
in charge/Site Manager/Project Manager/Vertical Manager and risk is to be assured in such
deviation. With due approval of the deviation, work should start.
6.7Final Checks & restoration of work area after completion of work :- Those checks to be
carried out by responsible supervisor in witness of his line hierarchy by use of specific checklist of
certain operational checks and once those completed satisfactory, PTW (if applicable) to be closed
and isolation arrangements to be restored by removing barricades/cautionary tags.
NO Deviation is allowed with respect to approved Risk Assessment & Method Statement. In case
of any difficulty for the practical reasons at any location, any deviation shall be discussed with site
in charge/Site Manager/Project Manager/Vertical Manager and risk is to be assured in such
deviation. With due approval of the deviation, work should start.

3. All tools & tackles are removed from the location.
4. No lose material is left on site.
5. Closing of work permit after co TATA POWER TROMBAY of activity.
6. Restoring of Barricading and caution Boards till TATA POWER TROMBAY of activity at that
location.
