Cause of Accident:
Accident of Train No.15633 UP Bikaner-Guwahati Exp. Between New
Domohani (NQH)-ltlaynaguri(NMX) Stations of Alipurduar Division of Northeast
Fronlier Railway at 16.53hrs on 13.01 .22 is classified as ,,Faiture of
Railway

Remarks & Recommendations


The railway shall create national data base of Locomotives
to capture its
historical data incruding major equipment on board and
fixture, record the
various inspection, schedule (Majoi and Minor) including
trip' inspection,
listory of failures, breakdownr and kirometres earned aft6r every tiip. ftre

official and logged into electronically. The data base shall be utilised for
failure analysis vendor education and approval and evolving maintenance
system based on kilometre earned by the locomotives.
ll. The traction motor which have completed 18 years of service shall be
withdrawn from the system and replaced on programmed basis. The life of
traction motor shall be reset to the original life prescribed i.e. 18 years.
lll. The traction motor of the same batch number as the failed one shall be
immediately withdrawn due to poor quality of welding'
lV. The breakage of nose of traction motor shall be detected electronically and
an alarm shall be generated which should automatically apply brake to the
train. This shall be implemented in passenger locomotives immediately.
V. A list of safety items shall be prepared for all type of Rolling stock which
shall be checked by Safety organisation of the Railway al workshops as
well as pit line examination centres to monitor quality of maintenance and
maintainers, this is also necessary since more and more activities by
workshops are being out sourced.
Vl. The loco links issued on Railways shall be examined to ensure that station
nominated for Tl schedule have requisite facilities and manpower to
carryout the inspection. Check shall also be performed by Safety
organisation of Railway to ensure that maintenance schedules of rolling
stock are adhered.
Vll. Railway shall make a review of inspection and maintenance facilities
available in routes electrified in last 4 years and ensure that the facilities
for inspection and maintenance of electrical loco are provided in time
bound manner.
Vlll. To ensure that Loco undergoes the maintenance schedule on time, it
should be made an standard item of monitoring in weekly safety meetings
done at GtV level.
lX. RDSO shall issue list of rejectable defects if found during inspection of TttI
received for rehabilitation the motor shall be condemned.
X. A loco safety certificate on the lines of Engine fitness certificate as
mandated in para 30304 of ACTM shall be issued after each inspection /
schedule. A copy of the safety certificate issued after trip inspection shall
be kept in the locomotive and shall be made part of the loco repair book. A
standard printed form shall be used in which all important safety items
shall be recorded. This shall be done for all locomotives working in the
Railway.
