Securities and Exchange Comission to transact business in the Philippines
under SEC License No. A200210139, with office address at 12F Ore Central
Building, 31st Street and 9th Avenue Fort Bonifacio Global City Taguig,
Philippines (the “Corporation”);

2. The Board of Directors of the Corporation unanimously adopted and


approved the following resolution on [insert date]:

“RESOLVED, That the Board of Directors of FIELDCORE


SERVICE SOLUTIONS INTERNATIONAL LLC – PHILIPPINES BRANCH (the
“Corporation”) authorize as it hereby authorizes the Corporation,
represented by its Resident Agent, Melinda Q. Cabase, to apply for
Accreditation as an Importer with the Bureau of Customs – Account
Management Office (BOC - AMO). The Applicant will be the main point
of contact between the company and the Bureau of Customs
throughout the validity of the company’s accreditation. The Applicant
shall receive notices from the Bureau of Customs regarding the
application for accreditation, and ensure that the Bureau of Customs
is notified of any changes in the information contained in the
application for accreditation, including change in business name;

“RESOLVED, FURTHER, that the Corporation authorize as it


hereby authorizes Melinda Q. Cabase, to sign the import entries
pertaining to the corporation.
