Dear Sir/Ma'am,

We are pleased to submit our formal quotation on the above subject as follows:

A. SCOPE OF WORK

Filjap Welding Institute shall provide the required manpower, equipment, materials and consumables necessary for welding tradetesting
of pipes/plates for employer/landbased recruitment agency/company screening to be conducted at 421 Palmera St cor Figueras St Brgy
402, Sampaloc City of Manila

B. COST OF PROPOSAL

For the implementation and completion of the above Scope of Work, Filjap Welding Institute shall charge the employes/landbased
recruitment agency/company addressed to this proposal as follows:

B.1 Facility Rentals

B.1.a Airconditioned VIP Lounge (Ground Floor)

B.1.c Holding Area (3 Rooms max of 20 persons per room)

B.2 Total amount of quotation stated above for this proposal.
B.3 The 12% Value Added Tax is NOT included in our price.

VAT in the Philippines is levied on almost all service and import sales, as well as the exchange, barter, or lease of goods or properties. The
tax is based on the gross selling price of properties and goods sold, or gross receipts from the sale of services, and is equivalent to a
uniform rate of 12%.

C. TERMS AND CONDITIONS

C.1 A Contract shall be issued in favor of Filjap Welding Institute prior to start of work; this proposal shall serve as a contract if
the “Conforme” portion hereunder is signed.
C.2 As per you our company policy, Customer’s Conforme and 50% down payment are required in processing of orders.
C.3 Plates and Pipes output after the tradetesting only have 15-calendar days leeway after the client has been notified to pick
up their plates and pipes, otherwise they will be deemed abandoned in favor of Filjap Welding Institute and Tradetest Center Inc.
C.4 No cancellation of order and down payment is non-refundable. Payment of the remaining balance shall be made on the day
of the actual testing.

D. VALIDITY

This quotation is valid only 5 days upon acknowledgement of receipt (if short notice please refer to expiry date of this quotation) otherwise
it shall be subject to our confirmation.
