Type 2 diabetes management education offers the basis to assist people with diabetes,
including guiding treatment decisions and activities, and has been demonstrated to enhance
care behaviors and knowledge, lower HbA1c levels, enhanced quality of life, decrease self-
reported weight, decreased costs of healthcare, and health coping (Chester et al., 2018). The
patient education is carried out in person via one-on-one teaching. To empower the patient and
support long-term adherence to their care plan, this project includes a user-friendly patient
teaching sheet as a necessary tool for type 2 diabetes self-management and education. The tool
A patient teaching sheet is an educational material with information about health topics,
treatments, and medications. A teaching sheet also incorporates answers to common patient
questions and concerns concerning those topics, treatments, and medications. The main purpose
of the patient teaching sheet is to provide the patient with knowledge and information concerning
the state of their health, treatment alternatives, and self-management approaches. The teaching
sheet is designed to educate the patient and assist them to be more active in rendering decisions

concerning their treatment and medication (Giuliani et al., 2020). They are symbolic of the
The content in the teaching sheet will be easy to understand and use, as well as informative
and enlightening. Therefore, it will be divided into five key sections. The first section will cover
understanding type 2 diabetes, where it will include a brief overview of type 2 diabetes, its
causes, risk factors, and prevention. The second section will cover the basics of healthy eating
and help the patient make healthier choices. It will also cover direction on developing balanced
diet including foods to limit and prioritize and portion sizes. The third section will cover the
benefits of staying active to manage type 2 diabetes. Recommendations for including physical
activity in patient's day-to-day operations, with examples of low-impact exercises appropriate for

factors that cause the patient's blood sugar to change across the day. It will also include
instructions for taking medications as prescribed. Lastly. This section will cover a set of
The patient should be able to both read and understand the teaching sheet. Most of the
average type 2 diabetes clinic's patient teaching sheets are written to a degree that is not readable
by a majority of patients. The teaching sheet is fabricated with the patient in mind, emphasizing
engagement and readability. Key elements include visual elements, which will help the patient to
retain information better. Infographics, images, and charts will be used to strengthen major

concepts. For instance, a plate diagram displays portion control. Accessible language is another
key feature. The patient teaching sheet is a vital tool for communicating information to the
patient; therefore, simple, jargon-free text will ensure the patient understands the information.
Another feature is the action-oriented tips, which will require the patient to take practical action
to manage type 2 diabetes. Every section will incorporate actionable guidance like "Aim for 40
The teaching sheet is deliberated to be a reference resource that the patient can store at
home and consult as required. Dealing with type 3 diabetes is not easy. The more concisely the
patient understands the condition, the more likely they will adhere to the intervention. The tool
will significantly enhance adherence to medication, increase quality of life, lower healthcare
costs, and reduce hospitalizations. The patient can use the tool to monitor daily levels of blood
sugar, plan meals, monitor their physical activity goals, and identify symptoms that would
The goal of the teaching sheet is to lead patients' well-being and health, focus on patient
self-efficacy, render informed decisions about care and treatment, and be committed to adhering
to treatments. Patient education is a vital tool that aims at empowering patients to become more
independent about the management of their health (Giuliani et al., 2020). Therefore, by
equipping the patient with a teaching sheet that is tailored to their needs, this tool promotes self-
efficacy and encourages active participation in the management of their condition. The teaching

sheet also corresponds with the extensive goal of the intervention of enhancing patient safety,
