accident program (ZAP) and the best practices based on QHSSE local and international standards as maybe
applicable. The findings from workplace safety assessments should be submitted to Top Management for
review and mitigation. This instrument is confidential, un-authorize distribution is prohibited.

SUMMARY OF ASSESSMENT:
As baseline for assessing timely development a joint SSHEQ Survey conducted. There have been some non-
conformity to standard and non-compliance to regulation cited and were due to lack of awareness on the High-Risk
Activities and Mandatory Requirements on SSHEQ in reference to RA11058 its IRR and the Labor Code and the
DENR-EMB Administrative orders.

ANNEX 2: NARRATIVE PROPOSAL FOR PRODUCTION/QUALITY
