AFFIDAVIT OF DISCREPANCY
(ONE AND THE SAME PERSON)

I, NAME, Filipino, of legal age, widow, and with residence and postal address
at Lagao, General Santos City, Philippines, having been sworn according to law, depose
and say:

1. That I was born on DATE to Spouses FATHER and MOTHER;


2. That I am a member of the Home Development Mutual Fund with Pag-IBIG
Number XXXX-XXXX-XXXX;
3. That in my Pag-IBIG records, what appeared in my full name was NAME and
there was no indicated middle name thereat;
4. That in my Philippine Statistics Authority Certificate of Live Birth and all other
public documents, my correct given name, middle name and last name was
correctly indicated;
5. That such discrepancy in my Pag-IBIG records was merely clerical during the
time of the registration;
6. That both names pertain to one and the same person as referring to me; and
7. I am executing this Affidavit to attest the truth of the foregoing specifically for
my documentary requirements for Pag-IBIG purposes and for whatever legal
purposes it may serve.
