Date At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.
The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
. The K to 12 Basic Education Program uses standards- and competency-based grading system. When we say
grading system we mean a system that educators use in a particular institution to evaluate each individual student’s
performance and learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be
attached to physical activities like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are
based on the weighted raw scores of the learners summative assessments. These summative assessment can be
classified into three components, namely, written works (WW), performance task (PT), and quarterly assessment These
three will be the bases for grading. The nature of the learning area defines the way these three components are
assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These are based on
learning standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical grades are used. The
minimum grade needed to pass a specific learner’s area is 60, which is transmuted to 75 in the report card. The lowest
rank mark that can appear on the report card is 60 for quarterly grades and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record in excel format
to facilitate high level of accuracy in the computation of the learners grade. Each learning area has its specific e-class
record. Aside from these, different DepEd forms are accomplished at varied time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They will be given
opportunities to examine and accomplish different forms, compute quarterly grades and final grades, and accomplish e-
class records aligned with their specialization.
Below are weights of the components for grades 1 to 10, and senior high school before the covid-19 pandemic.

The summary of learners progress is shown quarterly to parents and guardians through a parent-teacher conference, in which
the report card is discussed. The grading scale its corresponding descriptors, are in table 3.

Question:
Which of the three learners got a very satisfactory grade? No student got a very satisfactory grade. But the highest grade
was 91 which is outstanding.
Briefly describe the approach you used to systematically compute the grade of the learners.
For each learner, the scores from the four written works, and the four performance tasks are added together to get the total
score. The percentage score (PS) for written works is multiplied by 0.4 (40%), and in performance tasks is multiplied by 0.6
(60%). These two weighted scores are then added together to get the Initial grade. We then based the quarterly grade in their
equivalent transmuted grades based on Appendix B in DO No.8, s. 2015.

1. Which is easier to do, manually compute the grade of the students or the use of e-class record? Why?
Using the e-class record is often more convenient and efficient than computing grades manually. Because once set
up, e-class records save a huge amount of time, especially when revising grades or making changes. On the contrary,
manual calculations take time. E-class records organize data more systematically, making it easier to retrieve, sort, and
filter grades. Manual handling of records may result in data entry errors or loss of information. However, the simplicity
of using e-class records depends on knowledge of the system or program; once acquired, it is far more efficient than
manual methods.
2. Is it important to employ a systematic approach in computing the grades of students? Justify your answer.
Yes, using a systematic approach to determining student grades is important. This ensures that all students are
graded using the same criteria, promoting fairness. Inconsistent methods can lead to bias or prejudice, negatively
impacting student motivation and confidence.
Furthermore, a clear and consistent approach enhances transparency by explaining to both students and parents
how grades are determined. This provides a solid foundation for addressing any questions or concerns regarding
student grades. In essence, a systematic approach ensures a fair, transparent, and reliable grading process.
Completing various school forms is important for many reasons, including school governance, student progress, and
accountability. Attendance records, grading sheets, and persona data sheets are all examples of school forms that
contain important information about each student. They are useful for tracking academic progress, attendance, health,
and behavioral patterns over time. To comply with government regulations and educational policies, schools must keep
accurate student records. Forms such as enrollment records, report cards, and progress reports guarantee that the
institution meets those legal requirements.
Different school forms must be completed at different points throughout the academic year, each serving a specific
purpose. These purposes include getting to know our students better (like their contact info and any health needs) so
we can support them and keeping track of attendance daily to make sure they’re learning and safe. Completing these
forms on time helps us keep good records, communicate with families, and make sure we’re doing everything right for
our students.
5. What will happen if the school forms are not accomplished on the specified schedule?
Delays in completing school forms can create significant challenges for students, staff, and the overall school
operation. For example, incomplete registration o9r health forms may hinder the school’[s ability to provide essential
support and accommodations for students, particularly those with special needs or health concerns. Moreover,
failure to maintain accurate attendance records can lead to inaccurate tracking of student attendance. This can
impact school funding, and hinder identification of students at risk of academic disengagement.

1. If you are the teacher, what approach will you use to systematically compute the grades of you students?
Why do you prefer that approach.
I would rather use the using of Microsoft Excel and other application to compute my
students’ grades accurately and automatically. Additionally, this would help me to
reduce possible incorrectness of grades. Sometimes, manually computing the grades
will give you a higher possibility to compute it incorrectly. Moreover, there are some
reasons why I prefer to use digital application rather that doing it manually.
a. Easier to use.
b. Grades will automatically be computed by inputting scores and formulas.
c. It saves time.
d. It lessens the possibility to compute grades incorrectly.
e. One click, one grade.
f.You can make adjustment as long as you want.
g. It doesn’t necessarily need a lot of procedure to compute a grade.
3. Which of the different school forms is the most difficult to accomplish? What will you do to lessen the
difficulty encountered in accomplishing this form?
The SF 9 or the Learner’s Progress Report Card, with its other components and partial complications will be the
hardest to complete. It comprises a quarterly report on a learners’ academic, behavioral and attendance progress.
The most difficult part there is the calculation of each student’s grade individually, and such an exercise
cannot be completed without making a mistake and having to go through everything again. All you must do
to make this form easier to accomplish is to be careful and patient, which takes practice.
